眼科影像学检查对Vogt-小柳-原田病疗效的评价

摘　　要：目的 探讨眼B超、光学相干断层扫描（OCT）、眼底荧光血管造影（FFA）、吲哚青绿眼底血管造影（ICGA）对Vogt-小柳-原田病（Vogt-Koyanagi-Harada Disease,VKH）疗效评价的意义.方法 对1998年1月～2006年6月在本院确诊、治疗和随访的80例VKH患者的资料进行回顾性分析,所有患者均予以糖皮质激素治疗,在治疗前和治疗后不同的时间点进行眼B超、OCT、FFA、ICGA检查,对检测结果进行对比分析.结果 所有患者经过系统治疗后,炎症均得到了控制,患眼视力明显提高.治疗后,B超示玻璃体混浊和脉络膜增厚减轻至正常,OCT示视网膜神经上皮或色素上皮脱离逐渐平复,FFA示视网膜渗漏逐渐减轻至消失.治疗前ICGA示弱荧光黑斑、脉络膜灌注不良、脉络膜血管扩张、血管壁染色和渗漏、片状强荧光,治疗后上述病变逐渐恢复正常.结论 眼B超、OCT、FFA、ICGA对VKH疗效评价具有重要意义,为疾病的病情追踪、疗效评价和指导用药提供了客观依据,从而有利于对VKH的规范化治疗.Objective To investigate the clinical valve of B-uhrasonngraphy, optical coherence tomography （ OCT）, fundus fluorescein angingraphy （ FFA） and indocyanine green angiography （ ICGA） for the treatment of Vogt-Koyanagi-Harada Disease （ VKH ）. Methods A total of 80 patients diagnosed of VKH were treated with corticostemids. Before and after treatment, they were examined by B-uhrasonography, OCT, FFA and ICGA. Results The uveitis was controlled in all the patients. Before the treatment, B-ultrasonography feature were vitreous body opacity and chomids thickhess. After 6 weeks treatment,vitreous body opacity and choroids thickness decreased. Before the treatment, OCT were neurosensory retinal detachment and pigment epithelium detachment in all the patients, the average height of detachment was 612 ±70um,after 8 weeks treatment,the detachment disappeared. Before the treatment,FFA revealed muhifocal leakage in the pigment epithelium. After the treatment the retinal leakage decreased and disappeared. Before the treatment, ICGA revealed numerous patchy areas of hypofluorescence, choroidal filling defects, dilatation of choroidal vessels, choroidal hyperfluorescence. After the treatment all the manifestation normal but slowly. Conclusions B-uhrasonography, OCT, FFA and ICGA are able significant for the treatment of VKH. They are able to pursue the state of illness, appraise curative effect and coach using medicine.
