糖尿病患者白内障超声乳化吸除术后低视力原因分析

摘　　要：目的分析糖尿病(DM)患者超声乳化白内障吸除术后低视力的原因。方法分别于术前及术后1周、3个月,对36例行超声乳化白内障吸除术的DM患者(DM组)的手术眼、对侧未手术眼以及36例行超声乳化白内障吸出术的非DM患者(非DM组)的手术眼行荧光素眼底血管造影(FFA)检查和OCT检查。结果两组FFA检查结果:DM组:术前黄斑水肿2只眼(1例为双眼)(5.56%),术后1周黄斑水肿12只眼(33.33.00%),术后3个月黄斑水肿7只眼(19.44%);对侧未手术眼术后1周、3个月黄斑水肿1只眼(2.78%)。非DM组:术前无黄斑水肿,术后1周黄斑水肿2只眼(5.56%);术后3个月2只眼黄斑水肿消除。超声乳化白内障吸除术后DM组黄斑水肿的发生率明显高于非DM组。OCT检查结果:术前DM组黄斑水肿改变3只眼,术后3个月10只眼;非DM组术前黄斑水肿改变1只眼,术后3个月黄斑水肿改变1只眼,没有新的黄斑水肿发生。结论糖尿病白内障患者超声乳化吸除术后的黄斑水肿是影响术后视力的主要原因之一。Objective To investigate the reason of low vision in diabetic cataracts after phacoemulsification. Methods Thirty six eyes of cataract in diabetics were chosen randomly for measurement of macula using FFA （ fundus fluorescein angiography ） and OCT （ optical coherence tomography ） before phacoemulsification and after surgery. The other eyes in these patients and Thirty six eyes of cataract in nondiabetic patients with phacoemulsification were as control. Results The outcome of FFA In 36 eyes of diabetics with phacoemulsification ： 12 eyes had new macula edema after 7 days ,7 eyes had yet macula edema after 3 months, The other eyes in these patients had 1 eye happened macula edema after 7 days,3 months; In 36 eyes of nondiabetic patients with phacoemulsification ：2 eyes had new macula edema after 7 days, macula edema preclud after 3 months. The outcome of OCT in 36 eyes of diabetics with phacoemulsification after 3 months. 10 eyes had new macula edema after surgery,in 3 eyes with macular edema before surgery;Thirty six eyes of cataract in nondiabetic patients with phacoemulsification ： in 1 eye with macular edema before phacoemulsification, after surgery no new macular happens. Conclusion Macular edema is one of the most important causes of visibilities damage in diabetic tient patients after phacoemulsification.
