摘  要:目的探讨热休克蛋白70(HSP70)在经瞳孔温热疗法(TTT)治疗脉络膜新生血管(CNV)中的作用及机制。方法通过氪红激光眼底光凝诱导C57BL/6J小鼠30只(30只眼)产生CNV,6d后,观察眼底荧光血管造影(FFA),给予阈值上、阈值、阈值下TTT治疗及不治疗对照。正常C57BL/6J小鼠16只(16只眼),给予相应阈值上、阈值、阈值下TTT治疗。TTT治疗后7d,观察FFA,摘除眼球,做病理学检查和HSP70免疫组织化学检测。结果C57BL/6J小鼠CNV进行TTT治疗后7d,与未治疗对照组相比,阈值下和阈值TTT组CNV复合体明显变薄;阈值上TTT组CNV复合体未见明显变薄,新生血管腔无明显减少。正常眼和未经TTT治疗的CNV眼,HSP70阴性。实验性CNV和正常视网膜经TTT治疗后,HSP70免疫染色阳性可见于视网膜视锥视杆细胞层、外丛状层,染色范围取决于TTT能量大小。结论阈值下TTT治疗CNV,眼底镜下视网膜尚无明显变化时,HSP70在视网膜已有表达。HSP70在TTT抑制CNV的形成中起重要作用。TTT的能量、热休克蛋白的表达与CNV减少,三者之间的关系有待量化以指导临床。Objectiv To investigate the role of heat shock protein70 ( HSP70 ) in transpupillary thermotherapy (TTT) for choroidal neovascularization( CNV ). Methods CNV was induced in 30 mice( 30 eyes ) by krypton laser photocoagulation of the fundus. After 6 days, mice were examined by Fundus fluorescein angiography (FFA) and treated by TTT with a diode laser at surperthreshold ,threshold, subthreshold dose settings and some without any treatment. TTT was delivered to the normal retina of 16 mice( 16 eyes) accordingly. On day 7 after TTT , FFA were proform and eyes were examined by means of light microscopy and immunohistochemistry of HSP70. Results CNV was observed on day 6 after photocoagulation by FFA. On day 7 after TTT, the thickness of CNV complex was reduced in subthreshold and threshold TTT- treated mice, compared with untreated mice. However, CNV was not reduced obviously in surperthreshold TTT - treated mice, many. newly formed vessels still could be seen. Immunohistochemical staining for HSP70 could be seen in the photoreceptors at TTT sites, more prominent with larger dose TTT. Untreated control eyes showed no staining. Conclusion Despite the absence of changes evident by funduscopy, TTT resulted in does-dependent expression of HSP70. The induction of HSP70 may play a role in the reduction of CNV. Further experiments are required to determine how much HSP70 expression might reduce CNV.
