摘  要:目的:利用B和T淋巴细胞产生的抗原受体分子基因重排检测技术,探讨1例原发性脾脏恶性淋巴瘤(PLS)基因重排检测结果.方法:从组织蜡块中切取组织片5-10张,提取基因组DNA,PCR扩增,聚丙烯酰胺凝胶电泳后银染显示基因重排结果.结果:脾脏淋巴瘤免疫组化证实为B细胞性滤泡性淋巴瘤.从24个不同部位取得的组织经PCR均扩增成功,所有部位组织T细胞受体基因重排阴性,22个部位组织B细胞单克隆性免疫球蛋白基因重排阳性,基因重排阳性率91.7%.结论:应用PCR法检测B淋巴细胞IgH和T淋巴细胞TCR基因重排可作为淋巴细胞来源的良、恶性病变的一种辅助诊断手段,对脾脏淋巴瘤有较高的诊断价值.AIM: To assess the significance of detecting the gene rearrangements of IgH and TCR in the diagnosis of primary mali$1aant lymphoma of the spleens (PLS). METHODS: Twen- ty-four samples were taken from different places of the PLS. Ge- nomic DNA was extracted, and amplified vht PCR. The products were resolved on denaturing polyacrylamide gels and visualized through silver staining. RESULTS: All samples were successful- ly amplified. Gene rearrangements of IgH and TCR were negative in 10 cases of reactive lymphoid hyperplasia. In all of the 24 sam- ples from PLS, gene rearrangements of TCR У and TCR 13 were not found, and IgH gene rearrangements were found in 22 samples (91.7%). There was remarkable difference between PLS group and benign group in gene rearrangment ( P 〈 0.05 ). CONCLU- SION: Detection of the gene rearrangements of IgH and TCR by PCR could be a kind of subsidiary means to diagnose the lympho- proliferative disease. It is very helpful in the diagnosis of splenic lymphoma.
