细胞因子诱导的杀伤细胞治疗中晚期胃癌的疗效

摘　　要：目的评价应用细胞因子诱导的杀伤细胞（CIK）过继免疫治疗晚期胃癌的有效性。方法用淋巴细胞分离液分离出单个核细胞,将诱导后的CIK细胞回输给晚期胃癌患者。采用流式细胞术分析CIK培养过程中与55例晚期胃癌患者治疗前后外周血中的T细胞亚群（CD3^＋,CD4^＋,CD8^＋）、CIK细胞（CD3^＋CD56^＋）及NK细胞（CD3^-CD56^＋）的变化,并应用生化比色法与免疫分析技术检测患者治疗前后血清恶性肿瘤相关物质（TSGF）、胃癌相关抗原（MG7-Ag）、糖类抗原19-9（CA19-9）和癌胚抗原（CEA）含量。结果CIK细胞对K562细胞的细胞毒活性在14-21d时达峰值。CD3^＋CD56^＋细胞由起初的（0.7±0.4）%上升到14d的（31.6±5.5）%与21d的（35.8±9.7）%。经CIK细胞治疗后患者的肿瘤标志物水平下降,而NK与T细胞亚群水平明显升高,且CIK升高尤为显著,同时提高了患者的生活质量。总缓解率61.8%。结论用CIK细胞回输治疗晚期胃癌患者可提高患者的细胞免疫功能,并改善其临床体征,无明显毒副作用。Objective To evaluate the efficacy of cytokine-induced killer（CIK） cells used in adoptive irnmunotherapy of advanced gastric cancer. Methods Fifty-five patients with advanced gastric cancer were enrolled in this study. Autologous peripheral blood mononuclear cells（PBMCs） were separated with lymphocyte separating medium, and the cultured CIK cells were transpoted to the patients. The measurements included the development of irnmunophenotype（CD3^＋ , CD4^＋ , CD8^＋ ）, CIK（CD3^＋ CD56^＋ ） and NK（CD3^- CD56^＋ ） in cultures with flow cytometry, and serum levels of TSGF, MG7-Ag, CA19-9 and CEA with colorimetry of biochemistry, enzyme-linked irnmunosorbent assay（ELISA） or electrochemiluminescent immunoassay（ECLIA）. Results Cytotoxic activity of CIK cells to K562 cells reached the peak value from the 14th to 21st day during culture. CD3^＋ CD56^＋ value of the 1st,14th and 21st culture day were （0.7±0.4）%,（31.6±5.5）% and （35.8±9.7）%, respectively. After treatment, serum tumor marker levels decreased significantly and the NK and T irnmunophenotype were increased with an improved quality of life. The total remission rate was 61.8%. Conclusion The adoptive irnmunotherapy with CIK cells increases cellular immunity function and improves clinical physical signs with limited side effects in patients with advanced gastric cancer.
