摘　　要：目的探讨Cerb-B-2、P27基因的表达与食管癌发生、发展及浸润、转移的关系。方法采用免疫组织化学S-P法检测60例食管鳞癌组织、32例癌旁不典型增生组织及60例正常食管黏膜组织中Cerb-B-2及P27蛋白表达。结果食管鳞癌组织中Cerb-B-2蛋白表达与癌的组织学分级、浸润深度及淋巴结转移密切相关(P<0.05);P27蛋白表达与淋巴结转移密切相关(P<0.05);在食管鳞癌癌变过程中Cerb-B-2蛋白表达在正常黏膜组织、癌旁不典型增生组织及癌组织中的表达率依次增高,分别为25.0%(15/60)、71.9%(23/32)、86.7%(52/60),组间比较有明显差异(P<0.05);而P27蛋白在正常黏膜组织、癌旁不典型增生组织及癌组织中的表达率依次降低,分别为88.3%(53/60)、62.5%(20/32)、31.7%(19/60),组间比较有明显差异(P<0.05),Cerb-B-2与P27在食管鳞癌中的表达呈负相关。结论Cerb-B-2、P27基因在食管黏膜上皮癌变过程中起重要作用;联合检测Cerb-B-2、P27基因蛋白可成为食管鳞癌早期诊断和判断预后的分子指标。Objective To explore the relationship of the expressions of Cerb-B-2, P27 proteins and the occurrence, development, invasion and metastasis in esophageal squamous cell carcinoma. Methods S-P immunohistochemical method was used to detect the expressions of Cerb-B-2, P27 proteins in 60 cases with esophageal squamous cell carcinoma specimens, 32 cases with adjacent atypical hyperplasia epitheli- um and 60 cases with normal esophageal epithelium. Results The expressions of Cerb-B-2 proteins were closely correlated with the tumor grade, infiltration and lymphatic metastasis in esophageal squamous cell carcinoma （ P 〈 0. 05 ） , P27 proteins were closely correlated with the lymphatic metastasis. The expression rates of Cerb-B-2 proteins increased in turn in carcinoma, adjacent atypical hyperplasia epithelium and normal esophageal epithelium specimens, were 25.0% （15/60） ,71.9% （23/32） , 86.7% （52/60） , respectively. And there was a significant difference in the comparison group（P 〈 0. 05 ）. The expression rates of P27 protein decreased in turn in carcinoma, adjacent atypical hyperplasia epithelium and normal esophageal epithelium specimens , were 88.3% （53/60） ,62.5% （20/32） ,31.7% （ 19/60）, respectively. And there was a significant difference in the comparison group（ P 〈0. 05 ）. There was negative correlation between the expressions of Cerb-B-2 and P27. Conclusions Both Cerb-B-2 and P27 played an important role in the canceration course of esophageal squamous cell carcinoma,the united examination Cerb-B-2 and P27 may become one of molecular indices in the early diagnosis and prognostic judgment of esophageal squamous cell carcinoma.
