摘 要:目的探讨激光光凝对增生型糖尿病视网膜病变(PDR)患者玻璃体和血清中血管内皮生长因子(VEGF)及色素上皮衍生因子(PEDF)的影响。方法40例PDR患者分成激光光凝前组和激光光凝后组,每组20例20眼,均行全视网膜光凝,若黄斑水肿联合格栅样光凝。在玻璃体注射曲安奈德或切割前抽取玻璃体,并抽取晨起空腹血。用酶联免疫吸附试验检测VEGF及PEDF的质量浓度并进行统计学分析。结果两组患者玻璃体中VEGF的质量浓度为(154.44±13.90)pg/ml和(57.625±18.613)pg/ml(P<0.01);PEDF的质量浓度为(50.2+±33.3)pg/ml和(641+±557)pg/ml(P<0.01)。血清中VEGF的质量浓度为(24.039±5.485)pg/ml和(22.133±4.68)pg/ml(P>0.05);PEDF的质量浓度为(835±122)pg/ml和(901±195)pg/ml(P>0.05)。结论PDR患者激光光凝后眼局部VEGF下调,而PEDF上调,这些变化可能是视网膜新生血管消退的分子机制。Objective Retinal photocoagulation is an effective way to remove new vessels in proliferative diabetic retinopathy (PDR). However, no enough report in term of the influence of photocoagulation on some factors related to PDR neovascularization. Present study was to discuss the effects of photocoagulation on vascular endothelial growth factor(VEGF) and pigment epithelium-derived factor (PEDF) in vitreous and serum in patients with PDR. Methods Forty cases(40 eyes) with PDR were assigned to pre-photocoagulation group (20 eyes of 20 cases) and post-photocoagulation group(20 eyes of 20 cases). All patients underwent panretinal photocoagulation, and some patients with macular edema received the combination of panretinal photocoagulation and grid pattern photocoagulation. Samples of vitreous and peripheral blood were collected at the beginning of vitreous injection of triamcinolone acetonide or vitreous surgery,respectively,and the level of VEGF and PEDF in these samples were measured by enzyme-linked immunosorbentassay. Results The concentration of VEGF in vitreous was( 154.44± 13.90) pg/ml and (57. 625 ± 18. 613 ) pg/ml, respectively, and that of PEDF was ( 50. 2 ± 33.3 ) pg/ml and ( 641± 557 ) pg/ml respectively in two groups ( P 〈0.01 ). The VEGF level in serum was (24. 039 ± 5. 485 ) pg/ml and ( 22. 133± 4.68 ) pg/ml, respectively (P 〉 0. 05 ) , and that of PEDF was (835 ± 122)pg/ml and(901± 195 )pg/ml, respectively between two groups( P 〉 0. 05). Conclusion Level of VEGF decrease but PEDF increase in the eye with PDR after photocoagulation,indicating these may be the molecular mechanisms of removal of retinal new vessels.
