摘 要:目的:检测焦点黏附激酶(FAK)和基质金属蛋白酶2(MMP-2)在肝细胞肝癌中的表达,并探讨二者之间的关系。方法:将148例肝细胞肝癌、137例癌旁正常肝组织及10例正常肝组织制成组织芯片,直径为0.6mm;采用免疫组织化学SP法,研究FAK和MMP-2的表达情况。结果:148例肝细胞肝癌组织中FAK和MMP-2的阳性表达率分别为60.1%(89/148)和57.4%(85/148);137例癌旁正常组织FAK和MMP-2的阳性表达率分别为20.4%(28/137)和22.6%(31/137);10例正常肝组织中FAK和MMP-2的阳性表达率均为0%。两者在肝细胞肝癌组织中阳性表达率高于癌旁正常肝组织和正常肝组织(P<0.005);有门静脉分支癌栓组和包膜有侵犯组FAK阳性表达率均高于无门静脉分支癌栓组和包膜无侵犯组(P<0.005或P<0.01);有门静脉分支癌栓组、包膜有侵犯组及分化差组MMP-2阳性表达率均高于无门静脉分支癌栓组、包膜无侵犯组及分化好组。FAK的阳性表达与MMP-2的阳性表达有密切联系(P<0.005)。结论:FAK和MMP-2在肝细胞肝癌癌组织中表达明显增高,参与肿瘤的浸润和转移,且FAK和MMP-2在肝细胞肝癌癌组织中表达具有密切联系,可作为肝细胞肝癌治疗和判断预后的一个生物学指标。Objective To detect the expressions of focal adhesion kinase (FAK) and matrix metalloproteinase-2 (MMP-2) in hepatocellular carcinoma (HCC), and explore their correlation. Methods Tissue microarrays were generated from HCC tissues(148 cases), paracarcinoma tissues (137 cases), and normal liver tissues (10 cases). Each specimen on the tissue microarray was 0.6 mm in diameter. SP immunohistochemical method was used to measure the expressions of FAK and MMP-2, Results The positive rate of FAK expression was 60.1% (89/148) in HCC tissues, 20.4% (28/137) in paracarcinoma tissues, and 0% in normal tissues, while that of MMP-2 was 57.4% (85/148), 22.6% (31/137), and 0%, correspondingly. The level of expressions of FAK and MMP-2 in HCC tissues was higher than that in paracarcinoma or normal tissues (P 〈 0.005) ; The positive rate of FAK expression in the specimens with invasion to the branches of portal vein or with capsular infiltration was higher than that in those without invasion or without infiltration (P 〈 0.005 or P 〈 0.01 ), while that of MMP-2 in those with invasion to the branches of portal vein, or with capsular infiltration, or with poor differentiation was higher than that in those without invasion, or without infiltration, or with well differentiation (P 〈 0.005). FAK expression was closely related to MMP-2 expression (P 〈 0.005). Conclusions The level of expressions of FAK and MMP-2 is significant elevated in HCC, especially when invasion and metastasis occur. The expressions of FAK and MMP-2, closely related to each other, can be used as one of the biological indicators in the treatment and prognosis of HCC.
