联合脏器切除治疗晚期胃癌80例临床分析

摘　　要：目的探讨胃癌浸润周围脏器的联合脏器切除的适应证、手术方法以及并发症的防治。方法回顾性分析1995年4月至2005年5月我院80例胃癌直接浸润胰腺、脾脏、肝脏和横结肠而施行联合脏器切除治疗晚期胃癌病人的临床资料。结果1、3、5年总的存活率分别为70.1%(54/77),46.8%(36/77),20.7%(16/77)。主要并发症发生率为13.8%(11/80),手术病死率为2.5%(2/80)。手术并发症发生率和手术病死率与标准胃癌根治术相比差异无显著性(P>0.05),预后和标准胃癌根治术也相近。结论晚期胃癌的联合脏器切除术可以提高胃癌的切除率,改善患者的生活质量,提高5年存活率。Objective To investigate the indications and the surgical treatment of combined viscera resection for advanced gastric cancer,the prevention and treatment of complications. Methods The clinical data of 80 patients in our hospital with gastric cancer invading peripheral viscera and treated with enbloc resection combined other organs admitted between April 1995 and May 2005 were analyzed retrospectively. Results The overall 1 -, 3 - and 5 - year survival rate were 70.1% ,46.8% and 20.7%. The morbidity of main complication was 13.8%. The operative mortality rate was 2. 5 %. There was no significant difference in morbidity of complicantions and operative mortality between combined viscera resection of advanced gastric cancer and routine gastrectomy（P〉0.05）. The prognoses were similar in both groups. Conclusion Extended resection of advanced gastric cancer involving peripheral viscera improves the resectional rate,the quality of life and 5-year survival rate.
