Expressions and clinical significance of c-erbB-2, Cyclin D1, p16 and Ki-67 in hyperplastic disease of the breast

摘  要:目的:探讨c-erbB-2、Cyclin D1、p16和Ki-67与乳腺癌发生的关系。方法:通过免疫组化SP法检测20例乳腺单纯性增生、75例非典型增生(轻度20例、中度26例、重度29例)、28例导管原位癌及32例浸润性癌组织中c-erbB-2、Cyc-lin D1、p16和Ki-67的表达情况,并用正常乳腺组织作对照。结果:c-erbB-2的表达在轻度非典型增生与中度、重度非典型增生及导管内癌、浸润性癌比较差异均有统计学意义,χ2=19.131,P<0.001;中度、重度非典型增生与导管内癌、浸润性癌比较差异无统计学意义,χ2=5.660,P>0.05。Cyclin D1、p16和Ki-67的表达在中度非典型增生与重度非典型增生、导管内癌、浸润癌比较差异均有统计学意义(χ2值分别为23.660、27.726和20.782,P均<0.001),与单纯性增生、轻度非典型增生比较差异无统计学意义,P>0.05。在各组中,Cyclin D1、Ki-67与c-erbB-2呈正相关,与p16呈负相关,c-erbB-2与p16呈负相关。结论:c-erbB-2、Cyclin D1、p16和Ki-67表达异常发生在乳腺癌发生过程中的早期,其联合检测可作为早期诊断癌前病变和估计预后的有效指标。OBJECTIVE: To explore the relationship between c-erbB-2, Cyclin D1, p16, Ki-67 and progression of breast carcinoma. METHODS.. Immunohistochemistry was used to determinate the expressions of c-erbB-2, Cyclin D1, p16 and Ki-67 proteins in 20 cases of benign hyperplasia, 75 cases of atypical hyperplasia (mild,moderate, severe), 28 cases of ductal carcinoma in situ (DCIS) and 32 cases of invasive ductal carcinoma, which contrasted with normal breast tissue. RESULTS: The difference of the positive rates of c-erbB-2 protein expression were found between the group with mild dysplasia and the group with moderate or severe dysplasia, DCIS was significant, Χ^2 = 19. 131, P〈0. 001 ; but no significant differences were found between the group with moderate or severe dysplasia and the group with DCIS or invasive ductal carcinoma, Χ^2 =5. 660, P〉0.05. The difference of the positive rates of Cyclin D1, p16 and Ki-67 protein expressions between the group with moderate dysplasia and the group with severe dysplasia, DCIS or invasive ductal carcinoma was significant, Χ^2 = 23. 660, 27. 726, 20. 782, P 〈 0. 001; but there were no significant differences between the group with moderate dysplasia and the group with benign hyperplasia or mild dysplasia, P〉 0.05. The status of Cyclin D1 and Ki-67 protein was positively correlated with c-erbB-2, but negatively correlated with p16, and c-erbB-2 was negatively correlated with p16. CONCLUSION: The abnormal expressions with c-erbB-2,Cyclin D1, p16 and Ki-67 may be an early event in the progression of breast carcinoma, and combined detection of them could be used for early diagnosis of breast premalignancy and as an effective mark to monitor the prognosis.
