抑癌基因p27与肝癌临床病理及预后的关系

摘　　要：目的:探讨抑癌基因p27在原发性肝癌组织中的表达与临床病理及预后的关系。方法:运用免疫组化方法检测43例行根治性肝癌切除术患者病理组织切片中p27阳染指数(p27LI),分析其与患者临床病理及术后生存率的关系。结果:p27在肝脏细胞及癌细胞皆可表达,为胞核染色。其在肝癌细胞中的表达水平p27LI与肿瘤恶性程度及TNM分期呈负相关。单因素分析显示,高p27LI组患者术后中位生存期为28个月,低p27LI组为19个月,两者差异有统计学意义,χ2=5.8000,P=0.0112。多因素分析显示,p27为保护性独立预后因素,其回归系数(β)=-1.8011,危险度(Exp)=0.7734,P=0.0050。结论:p27作为抑癌基因,能调控肿瘤细胞的生长及凋亡,对行根治性肝癌切除术的患者具有独立预后价值。OBJECTIVE： To study the correlation between the expression of p27 and clinical pathological features. METHODS： In 43 patients＇ Hcc tissue sections, the p27 lable list （p27LI） was studied by the immunohistochmical method. Univiariate analysis,multivariate analysis （COX proportional hazard modle） and Χ^2-test were employed. RESULTS： p27LI was significantly lower in the poor differatiation and high classfication groups than in the other group. In univiariate analysis, the median survive time was significantly longer in high expression of p27 group than in low expression of one （Χ^2= 5. 800 0, P=0.011 2）. In multi-viariate analysis, p27LIβ= -1. 801 1, Exp=0.773 4,P=0.005 0. CONCLUSION： p27 gene plays a very important role in the cellular proliferationand differentiation of many malignant tumours, and it is an independent prognostic marker in HCC patients under resection.
