摘  要:目的:探讨耐药基因产物胸苷酸合成酶(TS)、谷胱甘肽-S-转移酶P i(GST-P i)、拓普异构酶Ⅱa(TopoⅡ)和增殖细胞抗原K i-67在恶性肿瘤组织中的表达及其临床应用。方法:应用免疫组化技术检测90例恶性肿瘤组织中TS、GST-P i和TopoⅡ和K i-67的表达,根据其结果和耐药机制指导临床化疗和药物适配分析。结果:总的阳性表达率TS48%,GST-P i69%,K i-67和TopoⅡ均为99%;而同一耐药基因产物在不同类型肿瘤中的阳性表达率TS为41%~100%,GST-P i为24%~91%,K i-67和TopoⅡ均为92%~100%。阳性细胞比例半定量TS是10%~60%,GST-P i和TopoⅡ均为10%~80%,K i-67是10%~90%。在药物适配分析中对两类药物敏感细胞比例之和≥110%的病例可达到99%。结论:组合检测TS、GST-P i、TopoⅡ和K i-67,基本上满足各类恶性肿瘤耐药基因检测的需要;以定性和百分比半定量结果指导临床化疗有一定参考价值。Objective: To study the protein expression and its clinical application of drug-resistance gene Thymidylate Synthase(TS), Glutatione S-Transferase Pi( GST-Pi), Topoisomerase Ⅱ a( Tope Ⅱ ) and growth cell antigen Ki-67 in malignant tumor tissue. Methods: TS, GST-Pi,Tope Ⅱ and Ki-67 protein expression in 90 cases of malignant tumor were detected by immunohistochemical assay. Acoording to the drug-resistance gene mechanism those results were used to directing individuality clinical chemotherapy and the pharmaceutical fitting analysis between the results and the drugs. Results: The total positive rate of TS was 48%, GST-Pi was 69%, both Ki-67 and Tope Ⅱ was 99% ; and positive rate of identical drug-resistance gene in different type tumors was that TS was 41% -100%, GST-Pi was 24%-91%, both Ki-67 and Tope Ⅱ was 92%-100% ; in half quantity of positive expression, TS was 10%-60%, both GST-Pi and Tope Ⅱ was 10%-80%, Ki-67 was 10%-90%. In the pharmaceutical fitting analysis, there were 99% cases whose summation of percent rade of sensitive cell 2 kinds of to drugs was more than 110%. Conclusion: Deatecting of TS, GST-Pi, Tope Ⅱ, and Ki-67 can satisfy the needs of drug-resistance gene detection in patients with all kinds of malignant tumor basically. A good individuality pharmaceutical fitting between the results of both quality and percent rate quantity and the drugs by using the results.
