摘 要:目的评价^(18)氟-胸腺嘧啶核苷(^(18)F-FLT)正电子发射体层(PET)-CT 显像对胸部肿瘤定性诊断的价值。方法对17例做了^(18)氟-脱氧葡萄糖(^(18)F-FDG)PET-CT 检查,但定性诊断困难的患者,在第2~3天进行了^(18)F-FLTPET-CT 显像。分析病变在两种不同显像剂 PET-CT 上的表现,分别测量二者的最大标准摄取值(maxSUV)。结果 8例恶性病变(5例肺癌、1例纵隔淋巴瘤、1例胸椎恶性肿瘤、1例胸椎转移瘤),其中7例见 FLT 异常摄取,5例肺癌平均 maxSUV为4.2(鳞癌2例、腺癌2例、肺泡癌1例);9例良性病变(5例肺结核、1例肺炎、3例纵隔淋巴结结核)无或轻度摄取 FLT,其中6例肺内良性病变平均 maxSUV 为1.6;9例良性病变中8例见 FDG 异常浓聚,其中6例肺内良性病灶平均 maxSUV 为3.9,3例纵隔淋巴结结核也见 FDG 明显摄取,平均 maxSUV 为11.0。11例肺内病灶 FDG 和 FLT 显像的敏感性、特异性、准确性分别为100.0%(5/5)、16.7%(1/6)、54.5%(6/11)和80.0%(4/5)、66.7%(4/6)、72.7%(8/11)。结论胸部肿瘤^(18)F-FLT 显像的特异性较高,在^(18)F-FDG 显像阳性,难以确定病变性质时,FLT 可以作为 FDG 的有益补充,二者联合显像有助于提高胸部肿瘤诊断的准确性。Objective To evaluate ^18F-fluorothymidine positron emission tomography (^18 F-FLT PET)/CT diagnosis value in chest oncology for ^18F-fluorodeoxygen-D-glucose (^18 F-FDG)PET-CT positive imaging of uncertain diseases. Methods 17 patients underwent ^18F-FDG PET-CT . After 2 or 3 days ^18F-FLT PET-CT scan was done, and the lesion differences were compared for two different tracers, measured maximum standard uptake value(maxSUV) in both of them. GE Minitrace and TracerLab FX-fn were used to automatically produce ^18 F-FDG and ^18 F-FLT. Results In 8 malignant tumor patients ( five lung cancers, one mediastinal lymphoma, one malignant tumor of thoracic vertebra, one metastatic cancer of thoracic vertebra),7 of 8 showed positive PET-CT ^18F-FLT results, average maxSUV was 4.2 in 5 lung cancer. Nine cases benign diseases found no or low-grade ^18 F-FLT uptake, average maxSUV was 1.6 in 6 pulmonary benign foci. In 9 benign lesion patients, 8 of 9 had 18 F-FDG uptake, average maxSUV was 3. Nine in 6 pulmonary benign foci. Three tuberculosis in mediastinum lymph can see obviously ^18F-FDG uptake, average maxSUV was 11. O. The sensitivity, specificity and accuracy of ^18F-FDG and ^18F-FLT imaging for pulmonary foci were 100. 0% ( 5/5 ), 16. 7% (1/6) ,54.5% ( 6/11 ) 和80. 0% (4/5) ,66. 7% (4/6) ,72. 7% ( 8/11 ), respectively. Conclusion ^18F-FLT has higher specialization in chest tumor, when ^18 F-FDG results is uncertain, ^18 F-FLT PET-CT scan provides useful supplementary information which assists chest tumor diagnosis.
