胃液中可溶性上皮型钙粘蛋白在几种胃疾病中的表达及其临床意义

摘　　要：目的探讨胃液中可溶性上皮钙粘蛋白(sE-cad)在检测几种胃部疾病中的诊断价值。方法113例因胃肠道症状接受胃镜检查配合活检确诊为慢性萎缩性胃炎16例,胃粘膜肠上皮化生20例,胃溃疡34例,胃癌22例,21例轻度浅表性胃炎(和无任何病理改变者作为正常对照组)。用双抗夹心酶联免疫吸附法检测胃液中sE-cad水平。结果胃癌组胃液sE-cad水平升高,与对照组比较差异有统计学意义(P<0.01);慢性萎缩性胃炎组及胃粘膜肠上皮化生组胃液sE-cad水平升高,与对照组比较差异有统计学意义(P<0.05)。结论胃液sE-cad水平升高可能对胃癌的诊断提供一定的佐证。Objective To explore diagnostic value of levels of sE-cadherin in gastric juice in differentiating gastric diseases. Methods Of 113 patients with gastrointestinal symptoms and diagnosed by endoscope and biopsy examination, 16 cases were chronic atrophic gastritis;20 cases were enteral metaplasia ;34 cases were peptic ulcer and 22 cases were gastrocarcinoma, other 21 cases were mild superficial gastritis or normal as controls. ELISA method was used to examine sE-cad contents of gastric juice of the all cases. Result： The gastric juice level of sE-cad were significantly higher in the gastric cancer groups than that in the other groups（ P 〈0. 01 ） . The gastric juice levels of sE - cad were also significantly higher in the chronic atrophic and enteral metaplasia groups than that in the control group （ P 〈 0. 01 ）. Conclusion The increase of sE- cadherin contents in gastric juice can possibly provide certain evidence to the gastric cancer diagnosis.
