腹腔镜下肾上腺肿瘤切除手术方式的探讨

摘　　要：目的:探讨腹腔镜下肾上腺肿瘤切除手术方式的选择。方法:总结分析自2003年3月～2007年6月139例腹腔镜下肾上腺肿瘤切除的手术方式及病理情况资料。结果:所有手术均采用经后腹腔途径,除1例改开放手术外,其余均顺利完成。行肾上腺全切除86例,肿瘤切除47例,根治性切除6例。1例双侧肾上腺皮质腺癌术后3个月死亡,2例肾上腺皮质腺癌术后5个月死亡,2例仍在随访,1例转移癌术后1年死亡,其余肿瘤随访2年,均无复发。结论:后腹腔镜下肾上腺肿瘤切除术是治疗肾上腺肿瘤的标准手术,术中应根据肾上腺本身结构及肿瘤的性质决定行肾上腺全切除或仅行肾上腺肿瘤切除术。To probe the surgical treatment of laparoscopic adrenalectomy. Methods：To summize the surgical treatment and pathology of 139 cases of laparoscopic adrenalectomy, from March 2003 to June 2007. Resalts： All of operations were successful with retroperitoneal laparoscopy, but lcase was converted to open surgery. Of all, 86 cases with total adrenalectomy, 47 cases with partial adrenalectomy, 5 cases with radical adrenalectomy. 1 patient with adrenocortical adenocarcinoma of two sides was dead three months later, 2 patients with adrenocortcal adenocarcinoma of one sides were dead five months later, 2 patients were still survived, 1 patient with transferred carcinoma was dead one year later. Other patients has been observed for two years and none recure. Conclusions：Retroperitoneal laparoscopic adrenalectomy is a standard treatment of curing adrenal tumer. We must select the manner of adrenalectomy ,according to the structure of adrenal and the character of tumor.
