摘 要:目的 观察应用双重粘弹剂(内聚性及弥散性)在硬核白内障超声乳化摘除中的临床疗效。方法 对38例51眼硬核白内障行超声乳化摘除术,术中联合应用内聚性及弥散性粘弹剂作为观察组;36例45眼术中单纯使用内聚性粘弹剂硬核白内障患者作为对照组。观察术后1d、2w视力,术后1d角膜水肿情况,术前及术后2w角膜内皮细胞计数。结果 观察组术后1天平均矫正视力0.53±0.21,角膜水肿发生率29.4%;对照组术后l天平均矫正视力0.31±0.18,角膜水肿发生率68.9%。两组间均有统计学意义(P〈0.01)。观察组术后的角膜内皮细胞丢失率5.1%,对照组14.7%。两组术中平均超声能量及时间,术后2w视力对比无统计学意义(P〉0.05)。结论 在硬核白内障超声乳化摘除中应用双重粘弹剂,可有效减少超声及液流等因素对角膜内皮细胞的损伤,从而减少术后角膜并发症的发生。Objective To observe the clinical effect of bilviscoelastic materials (dispersive and cohesive) in hard nuclear cataract endocapsular phacoemulcification. Methods 51 eyes of 38 hard nuclear cataract patients undergoing endocapsular phacoemulsification with dispersive and cohesive viscoelastic materials were study group. 45 eyes of 36 hard nuclear cataract patiets undergoing endocapsular phacoemulsification with cohesive viscoelastic materials only were control group. The visual acuity in one day and two week postoperation were measured. The endothelial cell density were measured in one week pre-operation and two week postoperation. The corneal edema was observed. Results The mean best corrected visual acuity was 0. 53 ± 0.21 and the rate of coneal edema was 29.4% in one day postoperation in the study group. The mean best corrected visual acuity was 0.31± 0.18 and the rate of corneal edema was 68.9% in one day postoperation in the control group. There was statistically significant differences in two groups(P〈0.01). The endothelial cell density lose 5.1% in the study group and 14.7% in the control group. There were no significant differences between two groups in pro-operation corneal endothelial cell density, phacoemulsification time and energy during operation, best corrected visual acuity two weeks post-operation(P〈0.05). Conclusions Bilviscoelastic materials in hard nuclear cataract phacoemulsification could protect corneal endothelium by decrease ultrasonic and fluid, it could decrease coneal complications.
