老年抑郁症状危险因素的1∶2配比病例对照研究

摘　　要：目的：探讨社区老年人抑郁症状发生的可能危险因素.方法：在现况调查的基础上,对西安城市社区90例有中重度老年抑郁症状和180例无抑郁症状者进行1∶2配比的病例对照研究,采用检验、条件Logistic回归等方法进行数据分析.结果：单因素分析显示,社会保障、居住模式、兴趣爱好、体育锻炼、负性生活事件、躯体疾病等因素在病例组和对照组之间有显著性差异（P〈0.05）;多因素分析证实去除了年龄、性别、居住地的影响后,负性生活事件、社会支持、生理功能、经济收入、体育锻炼仍与老年抑郁症状的发生显著相关,OR值（95%CI）分别为18.855（5.578～63.731）,5.005（1.924～13.021）,4.362（1.723～11.044）,4.195（1.137～15.477）,3.322（1.317～8.378）.结论：有负性生活事件、社会支持不良、生理功能差、经济收入低及不参加体育锻炼为老年抑郁症状发生的重要危险因素.AIM： To explore the possible risk factors of elderly depression in urban communities. METHODS： On the basis of investigation on current situation in elderly depression, a casecontrol study including 90 elders with depression and 180 nondepressive elders in Xi＇an urban communities was conducted. The cases and controls were matched in age, sex and inhabitancy. Data were analyzed by statistical technique of Chi-square test and conditional logistic regression. RESULTS： Univariate analysis showed that social security, living mode, interests, physical exercises, negative life events and diseases had significant differences between the 2 groups（ P 〈 0.05 ）. Multivariate analysis confirmed that the negative life events, social support, physical functioning, earning and physical exercises were significantly related to elderly depression,and their 0R（95% confidence interval）were 18. 855 （5.578 ～63.731）, 5.005 （1.924 ～13.021）, 4.362 （1.723 ～ 11. 044）, 4. 195 （ 1. 137～ 15. 477 ）, 3. 322 （ 1. 317 ～8. 378 ）, respectively. CONCLUSION： The negative life events, poor social support, bad physical functioning, low earning and physical exercises insufficiency were important risk factors of elderly depression.
