西方型、亚洲型多发性硬化与肿瘤坏死因子α水平的相关性研究

摘　　要：目的探讨肿瘤坏死因子α(TNF-α)水平与西方型、亚洲型多发性硬化(MS)的相关性。方法应用双抗体夹心酶联免疫法(ELISA)测定58例MS患者(其中西方型22例,亚洲型36例)和79名健康人血清TNF-α水平,并对MS组患者的神经功能缺损程度进行扩展病残状态评分(EDSS)。结果急性期MS组TNF-α水平为26.892±14.65pg/ml,较健康对照组(11.17±8.41pg/ml)显著升高(P<0.01),而缓解期MS组TNF-α水平(15.76±12.72pg/ml)与健康对照组比较无显著性差异(P>0.05)。急性期MS组TNF-α水平明显高于缓解期MS组(P<0.01);亚洲型MS急性期TNF-α水平(27.63±15.81pg/ml)明显高于缓解期(12.40±4.67pg/ ml,P<0.01),而西方型MS急性期TNF-α水平(25.49±12.65pg/ml)与缓解期(19.87±17.97pg/ml)比较无显著性差异(P>0.05)。两亚型急性期、缓解期TNF-α水平与EDSS评分均无显著相关性(P>0.05)。两亚型MS组间的EDSS评分比较无显著性差异(P>0.05)。结论不同病期、不同亚型MS患者血清TNF-α水平不同,但EDSS评分与TNF-α水平无明显相关性。Objective To investigate the relationship of the serum level of tumor necrosis factor alpha （TNF-α） with the Western-and Asian-type of multiple sclerosis （MS）. Methods A case-control research was performed on 58 MS patients and 79 healthy individuals as control group. Levels of TNF-α were determined by means of sandwich enzyme-linked immunosorbent assay （ELISA）. The neurologic impairment status of patients was scored according to the Expanded Disability Status Scale （EDSS）. Results The serum levels of TNF-α increased significantly in the patients with MS in acute stage compared with those in the healthy controls （26.89±14. 65pg/ml vs 11. 17±8. 41pg/ml, P〈0. 01）, but not in the patients with MS in remission （15. 76±12. 72pg/ml vs 11.17±8. 41pg/ml, P〉0. 05）. The serum levels of TNF-α in the patients with MS in acute stage were higher than those in the patients with MS in remission （26. 89±14. 65pg/ml vs 15. 76±12. 72pg/ml, P〈0. 01）, the same results were found in the patients with Asian-type MS （27. 63±15. 81pg/ml vs 12. 40± 4. 67pg/ml, P〈0. 01）, but not in the patients with Western-type MS （25. 49±12. 65pg/ml vs 19.87±17. 97pg/ml, P〉0.05）. There were no significant correlation between EDSS scores and the TNF a levels in the patients in acute stage both of Western- and Asian-type MS, and the same results were found in the patients in remission of both Western- and Asian-type MS. There was no significant difference in the EDSS scores between the two subtypes （4.1±1. 7 vs 4.4±2.3, P〉0.05）.Condusions The TNF-α levels differed among the stages of disease and MS subtypes, but no marked relation marked between the TNF-α level and the EDSS score.
