摘  要:目的比较冷牙胶侧方加压充填技术和高温热塑充填技术根尖封闭能力。方法选用40颗离体前磨牙,随机均分为A、B两组。A组:采用冷牙胶侧方加压法;B组:采用高温热塑充填法。A、B两组根管封闭剂选用氧化锌丁香油糊剂或AHplus。对照组只充填牙胶而不使用任何根管封闭剂。根管充填后的牙用1%亚甲蓝染色3d后,测量染色线长度,比较微渗漏情况。结果使用封闭剂的A2+A3组,染色线长度的中位数为3.25mm;使用封闭剂的B2+B3组,染色线长度的中位数为1.38 mm,两组之间差异有显著性(P<0.01)。对照组:染色线长度的中位数为6.00 mm,A2+A3、B2+B3组和对照组3组间染色线长度差异有显著性(P<0.01)。选用封闭剂氧化锌丁香油糊剂(A2+B2)组,染色线长度的中位数为4.25 mm;选用封闭剂AHplus(A3+B3)组,染色线长度的中位数为1.00 mm,两组之间差异有显著性(P<0.01)。结论高温热塑充填技术比传统冷牙胶侧方加压充填技术对根尖更具有良好的封闭效果。单纯充填牙胶的对照组根管微渗漏大于选用根管封闭剂的实验组,建议使用封闭性能较好的根管封闭剂。Objective To compare the apical sealing ability of two canal obturation techniques with different root canal sealers. Methods Extracted forty human premolar teeth were divided randomly into two experimental groups of 20 samples each and two control groups. Group A was obturated with cold lateral condensation, group B was obturated with high-temperature thermoplasticized gutta-percha. The experimental teeth of each group were filled with .Zinc oxide-eugenol paste, or AHplus respectively. The control groups were obturated with cold lateral condensation or high-temperature thermoplasticized gutta-percha without sealer. After dyeing for 3 days in 1% Methylene Blue, dyeing line were measured to reflect the microleakage of two canal obturation techniques with different root canal sealers and without sealer. Results Dyeing lines of group A2 + A3 had a median length of 3.25 ram, which was significantly different to that of group B2 + B3 ( group B2 + B3 1.38 mm,P 〈0.01 ). Dyeing lines of the control group had a median length of 6. 00 ram. The microleakage was significantly different among Group A2 + A3 , B2 + B3 and the control group(P 〈0. 01 ). The microleakage was significantly different between the two test groups (zinc oxideeugenol paste A2 + B2 4. 25 mm,Ahplus A3 + B3 1.00 mm,P 〈0.01 ). Conclusion High-temperature thermoplasticized gutta-percha showed better apical sealing ability compared with cold lateral condensation. The microleakage of the filled roots without sealer was more serious than that of the experimental roots filled with sealers. It is suggested that root canal sealers with better apical sealing ability must be used.
