高度近视眼多焦视网膜电图和传统视觉电生理改变与中低度近视的对比研究

摘　　要：目的:观察不同程度视网膜脱离高危眼的高度、中低度近视的视觉电生理改变,了解视功能损害程度。方法:回顾我院门诊高度近视的24例48眼,与高度近视伴对侧视网膜脱离眼20眼、中低度近视伴对侧视网膜脱离眼11眼、正常人48眼对照组进行传统视觉电生理F-ERG、F-VEP、多焦视网膜电图(一阶反应,firstorderkernel,FOK)联合检测。结果:传统视觉电生理检查,与中低度近视相比,高度近视眼各项F-ERG中,仅最大反应a波,单次视锥、30Hz闪烁b波表现为振幅下降明显,而高度近视眼伴对侧视网膜脱离的F-ERGb波各值下降明显(P<0.05)。多焦视网膜电图检查,中低度近视眼与高度近视眼在3-6环a波潜伏期延长、b波4环振幅和6环潜伏期有显著性差异(P<0.05),但与高度近视伴对侧视网膜脱离眼3-6环中心凹振幅密度、a波潜伏期和b波振幅及潜伏期各值均有显著性差异(P<0.05)。结论:中低度近视眼对侧伴视网膜脱离的在FERG、FVEP尚无改变的情况下,MERG已有多项变化,而这些变化依次轻于高度近视组和高度近视伴对侧视网膜脱离眼组;未发生视网膜脱离时,MERG潜伏期延长,特别是周边潜伏期的指标敏感,该指标的监控有利于视功能损害的亚临床诊断和预防性治疗,以减少高度近视眼视网膜脱离的发生。AIM： To observe vision electrophysiological changes in high and lower myopia eyes with high risk factor of retinal detachment preoperatively. METHODS： Fourty-eight eyes with high myopia were detected by vision electrophysiological examination of F-ERG, F-VEP and M-ERG of first orderkernel （FOK）, compared with 11 lower myopia eyes, 20 eyes of high myopia with the contralateral eyes of retinal detachment and 48 normal eyes. RESULTS： F-ERG amplitude and latent period changes descended obviously between the three control groups （P 〈 0.05）. F-VEP P100 and M-ERG latent periods in both eyes with high myopia descended compared with the lower ones （P 〈0.05）. CONCLUSION： There were significant differences on M-ERG values among the eyes with lower myopias, the eyes with the opposite retinal detachment and normal eyes. So the monitoring of M-ERG value is beneficial to the sub-clinical diagnosis and prevention of visual function damage to decrease the occurrence of retinal detachment in high myopia eyes.
