透明角膜切口白内障超声乳化术在抗青光眼术后的应用

摘　　要：目的:探讨抗青光眼滤过手术后进行透明角膜切口白内障手术的方法及手术对滤过泡的影响。方法:对抗青光眼小梁切除术后白内障患者45例(45眼),采用颞侧透明角膜隧道切口进行超声乳化白内障吸除及人工晶状体植入手术,同时对固定的小瞳孔进行扩张,对虹膜后粘连的瞳孔进行钝性分离。结果:白内障术后患者视力不同程度提高,视力≥0.5者占80%;术后3眼发生短暂性高眼压,经按摩后恢复正常,其余眼压均在正常范围;术前术后同1眼因青光眼术后7a滤过泡出现瘢痕化(2.2%),与术前比较差异无显著性(P>0.05)。结论:对抗青光眼滤过术后的白内障患者进行颞侧透明角膜隧道切口超声乳化白内障吸除术,可提高视力,同时不影响原有滤过泡功能。AIM： To study the methods of cataract ultrasonic emulsification through lucid cornea incision after anti-glaucoma filtering operation and the effects on filter vesicle. METHODS： In 45 cases （45 eyes） of patients with cataract undertgoing glaucoma trabeculectomy, the ultrasonic emulsification aspiration and implantation of artificial lens by temporal lucid cornea incision were carried out, the fixed small pupils were dilated and the adhered pupils behind sclera were bluntly separated. RESULTS： The vision of the patients receiving cataract operation was improved by different degrees. The 80% patients＇ vision ≥0.5. Temporal ocular hypertesion occurred in 3 cases and recovered to normal after massage. The rest of ocular pressures were all normal. Scar was formed in one eye＇s filter vesicle after 7 years of glaucoma operation, with no distinct differences compared with preoperation. CONCLUSION： The cataract ultrasonic emulsification through temporal lucid cornea incision after anti-glaucoma filtering operation can improve the vision without effects on the primary filter vesicle functions .
