胃癌患者外周血中骨桥蛋白的含量分析及临床意义

摘　　要：目的探讨骨桥蛋白（OPN）在胃癌患者外周血中的含量及临床意义。方法采用酶联免疫吸附试验双抗体夹心法检测51例胃癌患者（胃癌组）、29例慢性萎缩性胃炎患者（慢性胃炎组）、31例健康志愿者（对照组）、28例行胃癌根治术者（根治组）和29例胃癌术后6～12个月的随访者（随访组）外周血中OPN的含量。结果胃癌组患者外周血中OPN含量（540.44±223.47）μg/L,慢性胃炎组（331.59±127.11）μg/L,对照组（291.80±138.11）μg/L。胃癌组与慢性胃炎组、对照组比较差异有统计学意义（P〈0.05）;慢性胃炎组与对照组比较差异无统计学意义（P〉0.05）。根治组患者,外周血中OPN含量术前为（538.31±232.85）μg/L,术后为（278.67±152.93）μg/L,差异有统计学意义（P〈0.05）。随访组中10例术后复发、转移、死亡患者外周血中OPN含量为（627.10±117.47）μg/L,19例未出现复发、转移者外周血中OPN含量为（396.84±149.68）μg/L,两者比较差异有统计学意义（P〈0.05）。胃癌患者外周血中OPN的含量与胃癌的临床分期明显相关,而与性别、年龄、肿瘤大小及分化程度无关。结论检测外周血中OPN的含量对胃癌的诊断、治疗、预后评估及监测病情的进展有重要意义。Objective To investigate the level and clinical significance of peripheral blood osteopontin（OPN）of patients with gastric cancer. Methods The level of peripheral blood OPN was detected in 51 patients with gastric cancer（gastric cancer group）,29 patients with chronic atrophic gastritis（chronic gastritis group）,31 normal people（control group）,28 patients with gastric cancer treated by radical correction （radical correction group） and 29 patients followed group with gastric cancer which had been followed up for 6～12 months by ABC-ELISA method. Results The level of peripheral blood OPN was （540.44±223.47）μg/L in gastric cancer group,（331.59±127.11）μg/L in chronic gastritis group and （291.80±138.11）μg/L in control group. It was significant higher in gastric cancer group than that in control group and chronic gastritis group（P〈0.05）,it had no difference between chronic gastritis group and control group（P〉0.05）. The level of peripheral blood OPN before operation in radical correction group was（538.31±232. 85） μg/L and（278.67±152.93）μg/L after radical correction,and it had significant difference（P〈0.05）. The level of peripheral blood OPN in 10 patients with gastric cancer after radical correction patients but recurrence,metastasis and death was（627.10±117.47）μg/L,but in 19 patients who had no recurrence,metastasis was（396.84±149.68） μg/L （P〈0.05）. The level of peripheral blood OPN in gastric cancer patients was correlated with clinical stage,but had no correlation with the sex,age,size of tumour and degrees of differentiation. Conclusion The detection of peripheral blood OPN of patients with gastric cancer has greatly clinical significance in gastric cancer patients＇ treatment,diagnosis,prognosis evaluation and monitoring.
