摘 要:目的评价超声造影在肝肿瘤诊断中的价值。方法对50例肝实质性肿块采用常规超声和超声造影诊断进行分析,并与病理对照。结果常规超声和超声造影诊断肝恶性肿瘤的敏感性分别为58.3%、91.7%、特异性为94.4%、100%、准确性为70.4%、94.4%。造影后,除2例少血供胃肠道肝转移癌、1例手术后瘢痕及1例炎性假瘤未见增强外,其余肿块均有不同程度的增强。结论实时灰阶超声造影对肝肿瘤的诊断及鉴别诊断有重要的临床应用价值。Objective To evaluate the value of new contrast agent (SonoVue) and real time gray - scale contrast enhanced ultrasonography technology in the diagnosis of the hepatic tumors. Methods This study is based on the 50 clinical cases in which we have used color Doppler flow imaging (CDFI), power Doppler imaging (PDI) and real time gray -scale enhanced uhrasonography to examine the hepatic solid lesions. All lesions were confirmed by pathology diagnosis. Results The sensitivity for diagnosis of malignant tumor with uhrasonicgraphy is 58.3%, specificity is 94.4% and accuracy is 70.4%. Fifty lesions were variously enhanced on gray -scale uhrasonogram after injection of SonoVue except for 2 liver metastases, 1 case with 1 inflammatory pseudotumor of liver (IPL), and 1 case with 1 operational scar. The sensitivity, specificity, and accuracy in the diagnosis of hepatic malignant tumor with contrast enhanced uhrasonography were 91.7% , 100% and 94. 4% , respectively. Conduslon Real - time gray - scale contrast enhanced ultrasound can reflect the different symptoms in the different phases of the hepatic tumor, thus increase the diagnosis accuracy of hepatic tumors which convey the magnificent practical value to diagnose and discriminate hepatic tumors.
