VEGF MUC2表达及血清微转移对胃癌手术患者预后的影响

摘　　要：目的探讨血管内皮生长因子(VEGF)、粘蛋白MUC2及胃癌血清微转移对进展期胃癌手术患者预后的影响。方法采用免疫组化SP法检测49例胃癌标本中MUC2、VEGF的表达,并采用巢式RT-PCR法检测同一胃癌患者术前外周血中CK20mRNA及CEA mRNA的表达,并结合临床病理资料和随访结果进行分析。结果正常胃粘膜中VEGF、MUC2没有表达,MUC2在胃癌中的表达率为73.5%.VEGF在胃癌中的表达率为75.5%.在49例胃癌患者中,有31例发生血清微转移,微转移的发生率为63.3%.经多因素逐步Logistic回归分析,VEGF的高表达及病理组织学分型是影响胃癌患者的预后的独立因素(P<0.05)。结论VEGF及病理组织学分型是影响胃癌患者预后的独立因素。MUC2及微转移则与预后无关。Objective To investigate the relationship of the factor of MUC2 and VEGF and the micrometastasis with the prognosis in gastric cancer. Methods Forty-nine surgically removed specimens of gastric carcinoma were collected, MUC2 and VEGF were detected by immunohistochemistry, lmmunohistochemical staining was performed using SP methed. All the patients detected Ihe expression of CK20 mRNA and CEA mRNA in the peripheral blood before operation using nested reverse transcription palymerase chain reaction,any of them was positive,the Micrometastasis should be confirm. Results The positive expression rate of the MUC2 in gaslric cancer was 73.5% ;The positive rate of the VEGF in gastric cancer was 75.5% ; Both MUC2 anti VEGF were not expression in normal gastric mucosa. The positive rate of the micrometastasis in the gastric cancer was 63.3%. VEGF and types of cancers significantly associated with the prognosis of gastric cancer（ P 〈 0. 05 ）. Conclusion VEGF and types of cancers significantly associated with the prognosis of gastric cancer. MUC2 and the mierometastasis was not significantly correlated with and prognosis of gastric carcinoma.
