摘  要:目的研究高强度聚焦超声(HIFU)治疗乳腺癌后原发灶中肿瘤细胞的凋亡和增殖细胞核抗原(PCNA)的变化,探讨HIFU联合化疗药物对肿瘤的治疗效果。方法选取同期经病理确诊可手术的女性乳腺癌患者40例,分为HIFU治疗组和对照组各20例,HIFU治疗组患者术前接受化疗的同时进行一次HIFU治疗,对照组术前只接受化疗,两组患者均行乳腺癌改良根治性手术。应用原位末端标记法(TUNEL)及免疫组化SABC法,分别检测两组患者治疗前后乳腺癌原发灶中肿瘤细胞的凋亡指数(AI)和肿瘤组织的增殖细胞核抗原(PCNA)表达。结果HIFU治疗组化疗前AI和PCNA表达率分别为(0.57±0.09)%和(77.78±6.83)%,化疗后分别为(24.33±5.12)%和(30.23±4.51)%,化疗前后比较差异有显著性意义(P<0.05);对照组化疗前AI和PCNA表达率分别为(0.60±0.01)%和(76.34±7.27)%,化疗后分别为(10.67±3.43)%和(55.67±5.62)%,化疗前后比较差异有显著性意义(P<0.05);HIFU治疗组两种指标变化均较对照组明显,差异有显著性意义(P<0.05)。结论HIFU治疗能明显提高化疗药物的疗效,诱导肿瘤细胞凋亡并抑制其增殖发挥抗肿瘤作用,有利于乳腺癌的治疗。Objective To investigate the therapeutic efficacy of breast cancer treated with high intensity focused ultrasound (HIFU) in combination with adjuvant chemotherapy on the changes of cell apoptosis and proliferating cell nuclear antigen (PCNA) expression. Methods Forty operable female patients with pathologically confirmed breast cancer were divided into HIFU group and the control group randomly. Preoperatively, patients in HIFU group were treated with HIFU in combination with adjuvant chemotherapy, and patients in the control group were treated with adjuvant chemotherapy. All patients accepted radical improvement surgery. The apoptotic indices (AI) and proliferating cell nuclear antigen (PCNA) in tumor tissues were tested using TdT-mediated dUTP nick end labeling (TUNEL) and SABC immunohistochemical staining respectively. Results In HIFU group, AI and PCNA expression rate of pretherapy tumor tissues were 0. 57±0. 09% and 77. 78±6. 83o//oo respectively while they were 24.33± 5.12 % and 30.23±4.51% respectively after HIFU combinated with adjuvant chemotherapy. The differences were significant (P〈0.05). In the control group, AI and PCNA expression rate of pretherapy tumor tissues were 0. 60±0. 01% and 76.34±7.72% respectively while they were 10.67±3.43% and 55.67±5.62% respectively after adjuvant chemotherapy. The differences were significant (P〈0. 05). AI and PCNA expression rate changes were more significant in HIFU group than those of the control group (P〈0.05). Conclusions HIFU in combination with adjuvant chemotherapy can not only enhance the drug action of chemotherapeutic effects obviously, but also induce breast cancer cell apoptosis and restrain its proliferation in breast cancer, thus it will promote the therapeutic efficacy of breast cancer.
