长春瑞滨同步三维适形放疗治疗老年局部晚期非小细胞肺癌的临床研究

摘　　要：目的 评价长春瑞滨同步三维适形放疗治疗老年局部晚期非小细胞肺癌的疗效和毒性反应。方法 81例患者随机分为两组,化放组在三维适形放疗同时行化疗,长春瑞滨25 mg/m^2,静脉滴注,第1、8天,21 d为1周期,一般2～4周期。单纯放疗组：三维适形放疗。结果 化放组CR率17.5%,PR率50.0%,总有效率（CR＋PR）为67.5%;单放组CR率7.3%,PR率34.1%,总有效率为41.4%,两组差异有显著性（P〈0.05）。化放组和单放组的1,2年生存率分别为69.3%、69.3%和53.1%和16.7%,中位生存时间分别为17个月和11.2个月,差异有显著性（P=0.016）。结论 长春瑞滨联合三维适形放疗同步治疗老年局部晚期非小细胞肺癌安全有效,值得进一步临床研究。Objective To evaluate the efficacy and toxicity of vinorelbine and concurrent 3-dimensional conformal radiotherapy on elderly patients with locally advanced non-small -cell lung cancer. Methods Eighty-one patients were divided into the following groups ： group 1 was given to 3-dimensional conformal radiotherapy and concurrent chemotherapy （vinorelbine 25 mg/m^2 on day 1,8 of 21-day cycle, each patient should at least complete two cycles） ; group 2 was given to a total 3-dimensional conforma! radiotherapy. Resuits The overall response rate was 67.5% in group 1 and was 41.4% in group 2, respectively （ P 〈 0.05）. The survival rate was 69, 3% at 1 year, 69, 3% at 2 year, the median survival was 17 months in group 1 and compared with 53.1% , 16.7%, 11,2 months in group 2, respectively（P = 0. 016）, Conclusion Vinorelbine and concurrent 3-dimensional conformal radiotherapy on elderly patients with locally advanced non-small cell-lung cancer is a well-tolerated regimen in acceptable toxicity and will be an optimal therapy. Schems are required to improve disease control and overall survival.
