摘 要:目的探讨CK19和MC在肺癌患者胸水中的诊断价值。方法应用免疫细胞化学方法(S-P)研究44例肺癌患者胸水中的癌细胞和26例肺良性疾病胸水中的反应性间皮细胞的表达。结果CK19在肺癌患者胸水中的阳性率95.5%(42/44)明显高于在良性胸水中的阳性率7.7%(2/26),差异非常显著(P<0.01);而MC在良性胸水中的阳性率96.2%(25/26)明显高于在肺癌患者胸水中的阳性率22.7%(10/44),差异也非常显著(P<0.01);当CK19和MC联合应用时为最佳选择,其敏感性和特异性分别高达100%和88.5%。结论CK19和MC对肺癌患者胸水中癌细胞的诊断及鉴别诊断具有重要的临床应用价值。Objective To investigate the diagnostic value of CK19 and MC in pleural fluid of patients with lung cancer. Methods The expressions of CK19 and MC were examined by means of S-P immunocytochemical technique in cancer cells of pleural fluid from 44 patients with lung cancer and in reactive mesothelial cells of pleural fluid from 26 patients with benign lung diseases. Results The positive rate of CK19 in pleural fluid was 95.5% (42/44) from patients with lung cancer'and 7.7% (2/26) from patients with benign lung diseases,-showing a significant difference (P 〈 0. 01 ). MC was expressed 96. 2% (25/26) in reactive mesothelial cells of pleural fluid from benign lung diseases and, 22. 7% (10/44) in cancer Cells of pleural fluid from lung cancer, also showing a significant difference (P 〈 0. 01 ) . The optimal combination for assay was CK19 + MC: sensitivity and specificity were 100% and 88. 5% iespectively. Conclusion CK19 and MC are of important clinical value for diagnosing and differentially diagnosing the cancer cells in pleural fluid of patients with lung cancer.
