Array多焦点人工晶状体植入术后对比敏感度观察

摘  要:目的:评价白内障超声乳化吸除联合Array多焦点人工晶状体(IOL)植入术后对比敏感度(CS)的变化。方法:为46例(76眼)老年性白内障患者实施超声乳化吸除联合IOL植入术。术前随机将46例患者分为两组,18例(36眼)植入Array SA40Ne多焦点IOL(MIOL);28例(40眼)植入SI40NB单焦点IOL(SIOL)。术后1周、1月应用美国Stereo Optical公司产OPTEC3500视功能测试系统的对比敏感度测试包分别在空间频率为1.5cpd、3cpd、6cpd、12cpd和18cpd的昼及夜照明条件下对手术眼进行有眩光和无眩光的对比敏感度检查。所得数据进行t检验。结果:术后1周,MIOL组在各空间频率下CS普遍低于SIOL组,尤其在昼照明条件下。夜照明条件下,MIOL组在低、高空间频率下的CS均低于SIOL组,差异有统计学意义(P<0.05);昼照明条件下,MIOL组在低、中空间频率下的CS也均低于SIOL组,差异有统计学意义(P<0.01)。术后1月,两组间CS差异减小。除低空间频率下(1.5cpd)MIOL组CS低于SIOL组(P<0.05)外,其他测试状态下两组CS差异均无统计学意义(P>0.05)。结论:植入Array MIOL后对比敏感度降低主要体现在术后短期内(术后1周),该现象将随时间推移而改善。Objective: To evaluate contrast sensitivity(CS) in patients who had undergone uncomplicated phacoemulsifications with Array multifocal intraocular lens implantation. Methods: 46 patients (76 eyes) with phacoemulsification were randomly divided into 2 groups: 18 patients (36 eyes) implanted with multifocal intraocular lens (MIOLs, Array SA40Ne); 28 patients (40 eyes) implanted with monofocal intraocular lens(SIOLs, SI40NB). At 1 week and 1 month postoperatively, CS were measured separately with the Functional Acuity Contrast Tester(OPTEC 3500, Stereo Optical Co., USA) at 5 spatial frequencies (1.5, 3, 6, 12 and 18 cycles per degree, cpd) with and without a glare source under photopic ( 85 candelas per square meter, cd/m^2) and mesopic (3cd/m^2 )luminance level. The data were compared statistically using t - test. Results: At 1 week postoperatively, mean CS generally lowered in the MIOL group compared with SIOL group, especially under photopic conditions. The decrease of CS in MIOL group was mainly showed in low and high spatial frequency under mesopic conditions (P〈0.05) and in low and medium spatial frequency under photopic conditions (P〈0.01). At 1 month postoperatively, the difference of CS between two groups became smaller. The scores of mean CS in two groups did not differ significantly in almost all spatial frequencies (P〉0.05), excepting low spatial frequency(1.5 cpd) (P〈0.05). Conclusion: Reduced CS is found in the Array MIOL group only at 1 week postoperatively, which would be improved as time went on.
