摘  要:应用B超和磁共振对84例脑肿瘤患者进行视神经直径测量,分别计算在不同脑压、不同部位、不同肿瘤性质及不同病程时脑肿瘤的视乳头水肿及视经束增宽的发生率,探讨视神经增宽对脑肿瘤的早期诊断价值。结果示84例脑瘤患者视乳头水肿的发生率为68%,B超和磁共振测量视神经增宽的发生率为83%和89%(与视乳头水肿相比,P<0.05)。当脑压在1.97~2.50kPa、肿瘤为胶质瘤、肿瘤位于大脑半球及病程较短时,视神经增宽的发生率明显高于视乳头水肿的发生率(P<0.05)。结论:B超和磁共振测量视神经直径均能提高脑瘤的早期诊断,但B超操作方便、精确性强、经济有效,应为首选。To explore the value of increasing width of optic nerve in diagnosis of early stage of brain tumors, thediameters of optic nerves of 84 patients with brain tumors were measured with B type ultrasonic tomogtaphy and magneticresonance image (MRI), the rate of increasing width of optic nerve and the rate of papilledema were counted in different con-dition of intracranial pressure, tumor place, tumor type and the course of the disease respectively. We found that the rateof increasing width of optic nerve was 83% for B type ultrasonic tomography and 89% for MRI with higher papilledema rate(68%, P<0.05). In patients with 1. 97-2. 50 kPa intracranial pressure, glioma tumor localized in cerebral hemisphere andabout one month course of disease, the rates of increasing width of optic nerve were significantly higher as comparied withthat of P4pilledema (P<0.05). The results suggested that measurement of optic nerve with B type ultrasonic tomographyand MRI helps the diagnosis of brain tumors in the early stage, while B type ultrasonic tomography is more useful tecauseof its convenience practice, high precision and economy.
