高血压性脑出血患者血C-反应蛋白及TNF-α测定及临床意义

摘　　要：目的探讨高血压性脑出血患者发病早期血C-反应蛋白(CRP)及肿瘤坏死因子-α(TNF-α)含量变化及临床意义。方法随机选取102例高血压性脑出血患者,免疫比浊法测定发病24h内血CRP含量;放免法测定TNF-α含量,分析二者与病情变化、出血量及转归的关系。结果CRP和TNF-α阳性率分别为57.8%和66.7%;二者随临床神经功能缺损程度和出血量增加而逐渐增高(P<0.01);病情无好转者CRP及TNF-α阳性率明显高于病情好转者(P<0.01);CRP和TNF-α均阳性者预后比其中一项阳性者差(P<0.05)。结论血CRP及TNF-α可作为高血压性脑出血患者发病早期病情变化及早期转归的监测指标和评价指标,具有重要诊断价值。[ Objective] To observe changes of serum tumor necrosis factor-alpha（ TNF -α） and C-reactive protein（CRP） in patients with hypertensive cerebral hemorrhage, and their clinical significance. [ Methods] Randomly selected 120 cases of inpatients with hypertensive cerebral hemorrhage , and divided them into 3 groups ： mild group , medium group , severe group . Within 24 hours after stroke, the content of CRP in blood was measured by nephelometer immunoassay, and the content of TNF -α was measured by radioactive irmnunoassay. The positive rate of TNF -α and CRP were determined, the content of TNF -α and CRP in the three groups , the positive rates were compared. [ Results] Among the 102 patients with hypertensive cerebral hemorrhage , the positive rate of CRP and TNF -α was 57.8% and 66.7% respectively, and the positive rate increased with the increasing of neurological function deficit degree and hemorrhage （P 〈 0.01 ）. The difference of CRP and TNF -α in blood among the mild group , the medium group and the severe group were significant （P 〈0. 01 ）. The positive rates of CRP and TNF -α in the no-improved group were significandy higher than those in the improved group （P 〈 0. 01 ）. And the prognosis would be wooer in cases with CRP and TNF -α increased together than those with only one of CRP and TNF -α increased （P 〈 0. 05）. [ Conclusion] CRP and TNF -α may serve as the index of monitoring early change in patients with hypertensive cerebral hemorrhage, and evaluating early prognosis.
