RGP在大龄儿童屈光参差性弱视矫治中的应用

摘　　要：目的探讨硬性透气角膜接触镜(rigidgas-permeable contact lenses,RGP)在大龄儿童屈光参差性弱视矫治中的应用价值。方法选择20例(20眼)9～16岁屈光参差性弱视儿童作为RGP组,以28例(28眼)条件相似的儿童为框架眼镜组。两组均辅以正规弱视综合治疗,观察6个月,对RGP组首次配戴RGP前后的矫正视力及两组1、3、6个月后的视力变化、疗效进行统计学比较,同时观察RGP组的不良反应。结果RGP组首次配戴RGP时RGP矫正视力4.61±0.25,框架眼镜矫正视力4.41±0.23,差异有显著性(t=7.968,P<0.01);经弱视治疗后1、3、6月后,RGP组视力改善明显好于框架眼镜组,RGP组有效率分别为75%、80%、85%,框架眼镜组有效率分别为7.14%、21.43%、28.56%,两组同期疗效比较均具显著性差异(P<0.01,χ2检验)。在随访期间,未发现角膜上皮损伤或角膜感染等并发症。结论RGP在大龄儿童屈光参差性弱视矫治中具有积极的临床意义,与框架眼镜相比具有明显优势,为弱视治疗提供一种安全、有效、快速的矫治手段。Objective To observe the effectiveness and safety of using rigid gas-permeable contact lenses （RGP） on anisometropie amblyopia in older children and assess the clinical value of RGP. Methods 20 eases （20 eyes） older children with anisometropie amblyopia, age 9-16 years, were selected to wear RGP, and another 28 eases （28 eyes） of counterpart were selected as controls by wearing spectacles. Two groups received amblyopia therapy with a patch ,etc. The study was done over 6 months with follow-up visits every 1 months. Visual acuity was abserved from two groups and complications were monitored from the RGP. Results In RGP group, when wearing RGP for the first time, the corrected visual acuity with RGP （4.61±0.25） were better than that with spectacles （4.41±0.23）. The result was statistically significant（ P〈0.01, t-test）. 1, 3 and 6 months later, the total effective rate in RGP was 75%,80% and 85% respectively, while spectacle group was 7.14%,21.43% and 28.56%. There was significant difference between the two groups （P〈0.01,X^2-test） over the same time. In RGP group, No serious complications such as corneal abrasion and inflammation occurred within fllow-up period. Conclusion RGP is superior to spectacles , it can obtain more excellent optic corrections and better corrected visual acuity. RGP has good therapeutic effect and is a safe, effective And rapid means to improve visual acuity for anisometropic amblyopia.
