腹腔置管引流联合羟基喜树碱、鸦胆子油序贯给药治疗恶性腹腔积液

摘　　要：目的探讨腹腔置管引流联合羟基喜树碱、鸦胆子油序贯给药治疗恶性腹腔积液的疗效和安全性。方法选择72例伴有中、大量恶性腹水的晚期肿瘤患者，采用腹腔置管引流尽量放干腹水后，腹腔内注射药物羟基喜树碱20mg，地塞米松10mg，多巴胺60mg，速尿40mg．夹闭引流管6h后将鸦胆子油50mL注入腹腔，1周后重复，共2-4次。结果全组腹水完全缓解率为20％（15／72），部分缓解率为50％（36／72），腹水控制有效率为70％（51／72）。主要毒性反应是白细胞、血小板下降。结论腹腔置管引流联合羟基喜树碱、鸦胆子油序贯给药治疗恶性腹腔积液具有较好的疗效和安全性。Objective：It is to evaluate the efficacy and safety of intraperitoneal catheter combined with hydroxycamp-tothecin and seed oil of brucea javanica in advanced tumor patients with malignant ascites. Methods 72 advanced tumor patients with medium or abundant malignant ascites were chosen, and their malignant ascites was drained with intraperitoneal catheter. Then the patients were given hydroxycamptothecin 20 mg, Dexamethasonen 10 mg, Dopamine 60 mg, Furosemide 40 mg by intraperitoneal injection. Seed oil of brucea javanica 50 mL was used after the catheter was closed for 6 hours. It was repeated in 1 week, 2-4 times altogether. Results The complete response of ascites was 20% （15/72）, the partial response was 20% （15/72）, the response rate of acites was 70% （51/72）. The main side effects were neutropenia and thrombocytopenia. Conclusion Intraperitoneal catheter combined with hydroxycamptothecin and seed oil of brucea javanica is feasible and effective for advanced tumor patients with malignant ascites.
