全反式维甲酸、化疗及白血康序贯治疗急性早幼粒细胞白血病疗效观察

摘　　要：目的:观察全反式维甲酸、联合化疗及白血康序贯治疗急性早幼粒细胞白血病的疗效和副作用。方法:对经诱导达完全缓解的41例急性早幼粒细胞白血病(APL)患者,停药4周后采用HA、DA或MA方案巩固治疗,间歇4周后,给予口服白血康胶囊28天;再间歇4周给予口服全反式维甲酸28天,为1个周期。间歇4周再重复以上周期。完全缓解后第1年每疗程间隔1个月,第2年每疗程间隔2个月,第3年3个月。结果:45例APL患者诱导缓解后41例达完全缓解,40例参与序贯治疗者,3年和5年DFS和OS均为96.7%,毒副作用轻。结论:应用全反式维甲酸、联合化疗及白血康序贯治疗急性早幼粒细胞白血病,3年和5年DFS和OS均明显提高,这一新的联合治疗方案有可能使APL成为通过非骨髓移植手段获得治愈的白血病。To observe the curative effect and side effects of sequential therapy with all - trans retinoic acid, combined with chemotherapy and Bai-Xue -kang capsules on acute promylocytic leukemia （APL） patients. Methods： Total of 41 APL patients who had reached complete remission （CR） received combined with chemotherapy of HA, DA, or MA, then Bai - Xue - kang capsures for 28 days, and all - trans retinoic acid for 28 days one by one at 4 weeks interval respectively. Above sequential treatment termed a therapeutic circled over the first year ,then the interval between each treatment was extended to 8 weeks in the second year,12 weeks in third year. Results： The CR rate of 41 APL patients after remission induction was 91.11%. DFS and OS of 3 years and 5 years for 40 APL patients received sequential treatment were both 96.7% ,while side effects were mild. Condusion： With sequential therapy of all - trans retinoic acid, combined with chemotherapy and Bai - Xue - kang capsules, DFS and OS of 3 years and 5 years for 40 APL patients increased significantly. This novel strategy of combined treatment maybe make APL becomes the most probably healing-well leukemia through non-transplantation therapy.
