MUC1 MUC2与MUC4在胃癌组织中表达及临床意义

摘　　要：目的研究MUCI、MUC2、MUC4黏蛋白在胃癌组织中表达与临床病理学之间的关系。方法应用免疫组织化学(SP)法检测92例胃癌组织标本中MUC1、MUC2、MUC4在胃癌中的表达。结果MUC1的阳性表达率为83%(76/92),不同组织学之间的表达差异有统计学意义(P<0.05),在高分化腺癌中表达最强,表达程度与患者年龄、淋巴结有无转移密切相关;MUC2表达阳性率为84%(77/92),不同组织学类型肿瘤组织中的表达差异有统计学意义(P<0.05),在黏液腺癌中表达最强100%(7/7);MUC4的阳性率为41%(38/92),除性别差异外,与其他的临床资料无关。相关分析发现,MUC1的表达与MUC2的表达呈正相关。结论MUC1在不同组织类型的胃癌中表达不同,在高中分化腺癌中最高,并与淋巴转移有关;MUC2在不同的组织类型的胃癌中表达不同,其中黏液腺癌表达最强,可作为研究黏液腺癌的指标;MUC4在胃癌中的表达下降,可作为研究胃癌发生、进展的指标。MUC1和MUC2之间有协同作用。Objective To investigate the clinical signficance of the experssion of different kinds of mucins in gastric cancers and the relationship between them. Methods To detect the proteins by Immunohistochemical methods and to define the type of cancers according to the WHO criterion. Results The rate of mucl gene expression was 83% （76/ 92）, its expression was related with the type of the cancers （P 〈 0.05）, the age of patients, the involvement of lymph nodes, but there was no significant difference between the patients sex, the location of tumor and the metastasis to other organs. The rate of MUC2 gene expression was 84% （77/92） ,its expression is also related with types of cancers （P〈 0.05）, and 100 % mucinous carcinoma showed a strong expression, but there was no relationship within other clinical outcomes. The rate of MUCA gene expression was 41% （38/92）, but its expression was not related with any of the characteristies investigated at present. There was a positive relationship between MUC1 and MUC2. Conclusion MUC1, MUG2 are good markers for different types of gastric cancer. MUC1 gave a strongest expression in tubercular carcinomas, and is related to the involvement of lymph nodes. MUC1 is an excellent indicator for mucous carcinoma. MUCA is also a good marker in studying the progress of gastric carcinoma.
