摘　　要：目的通过对合并血管切除胰头癌和肝门胆管癌资料分析，探讨合并血管切除的可能性和必要性。方法总结我院自2003年4月至2006年12月间11例侵犯门静脉（PV）、肠系膜上静脉（SMV）、下腔静脉（IVC）等血管的胰头癌和肝门胆管癌的手术切除情况，介绍手术方法和技巧。结果无手术死亡，无相应并发症发生，手术时间及出血量均未显著延长和增加。结论对于无广泛血管浸润的胰头癌和肝门胆管癌，应积极考虑行合并血管切除的扩大根治术，有望改善生活质量，提高生存率。Objective By analyzing the data of combining vascula resection in carcinoma of pancreas head and hilar cholangiocareinoma , to evaluate the possibility and necessity of the resection. Method Summarled 11 cases of carcinoma of pancreas head and hilar cholangiocareinoma resection between April 2003 and December 2006 in our hospital ,who combined with portal vein（PV） , superior mesenteric vein（SMV） , inferior vena cave （IVC） invasion. And introduced the surgical method and skill. Results There was no operation death, no corresponding complication , the operation time and bleeding volume didn＇t significantly prolong and increase. Conclusion With regard to no extensive vascular invasion in carcinoma of pancreas head and hilar cholangiocareinoma, we should consider to undertake extending radical resection combined with vascula resection actively, in order to improve the survival quality and increase survival rate.
