硬性透气性角膜接触镜矫正青少年进展性近视的疗效观察

摘　　要：目的评价国产Boston RGPCL(硬性透气性角膜接触镜)矫正近视和散光的效果及对配戴者眼角膜曲率、角膜散光、高阶像差的影响。方法按自愿知情原则选择28例(55眼)16岁以下近视及散光的青少年配戴国产BOSTON RGPCL,半年后随访其眼的最佳矫正视力(BCVA)、屈光度、角膜曲率、高阶像差,并与配戴前进行比较。结果配戴RGPCL半年后最佳矫正视力有提高,但差异无统计学意义(χ2=3.909,P>0.05)。配戴RGPCL后近视屈光度半年平均增加(0.15±0.28)D(等效球镜),较配戴前明显降低(t=-24.79,P<0.05)。配戴RGPCL后角膜曲率K1、K2值均较配戴前变小,角膜有变平趋势,配戴前后差异均有统计学意义(t=2.087、3.396,均P<0.05);角膜散光(K1-K2值)与配戴前比较差异无统计学意义(t=0.860,P>0.05)。配戴前后眼总体高阶像差比较(包括总体高阶像差、彗差、三叶草、四叶草、球差及高阶散光)差异均无统计学意义(t=1.933、-1.518、1.246、-1.596、1.300、0.225,均P>0.05)。结论RGPCL对近视矫正效果良好,可起到阻止近视快速发展的作用,对总高阶像差无明显影响。Objective To evaluate the effect of Boston RGPCL on myopia and astigmatism correction and the influence to the higher order aberrations, corneal curvature and corneal astigmatism. Methods With informed-consent, 28 cases （under the age of 16 years） 55 eyes with myopia and astigmatism were selected and studied perspectively. The clinic data including the corrected visual acuity （ BCVA ）, degrees of myopia, the corneal curvature, higher order aberrations and follow-up records of 6 months were analyzed. Results After treatment with RGPCL, the corrected visual acuity by RGPCL was superior to that by glasses, but without any statistically significant difference （ X^2=3.909,P〉0.05 ）. Degrees of myopia increased by （0.15 ± 0.28 ）D ,significantly tess than before （t=-24.79, P〈0.05 ）. The corneal curvature K1 ,K2 both decreased than before, patients＇ corneas tend to be fiat. There were statistically significant difference （t=2.087,3.396, P〈0. 05 ）. There were not any statistically significant difference of corneal astigmatism （K1-K2）between two groups （t=0.860, P〉0.05）.The root mean square （RMS） values of higher order aberrations （include： RMS-high, RMS-coma, RMS-trefoil, RMS-4foil, RMS-sph and RMS-HiAstigmatism） has not any statistically significant difference between two groups （t=1. 933 ,-1.518,1.246 ,-1.596,1.300,0.225,p〉0.05 ）. Conclusion RGPCL has good therapeutic effect on abnormal ocular refraction .RGPCL can be used for teenager myopia to slow down myopia progression, and there is no influence on the high order aberrations.
