小脑囊性占位病变的MR诊断价值

摘　　要：目的：评价ＭＲ检查对小脑囊性占位性病变的诊断价值。材料和方法：４９例小脑囊性占位性病变均经手术和病理证实，其中男性２９例，女性２０例。年龄２～７９岁，平均４７岁。其中血管网状细胞瘤２２例，转移瘤１１例，星形细胞瘤７例，室管膜瘤４例，髓母细胞瘤３例，皮样囊肿１例，表皮样囊肿１例。结果：ＭＲ对小脑囊性占位性病变的发现率极高，具有较高的空间定位能力，不仅能全面观察囊性病变的形态，而且能通过Ｔ１加权Ｔ２加权扫描来观察病变的信号特点，周围脑组织的水肿程度以及增强扫描后病变的信号变化。结论：ＭＲ对小脑囊性占位性病变的诊断具有较高的准确性，尤其是增强扫描对定性诊断及鉴别诊断具有重要的价值。Purpose:To evaluated the magnetic resonance value in diagnosis of cerebellar cystic space-occupying lesions.Materials and Methods:49 cases of cerebellar cystic lesions were verified by operation and pathology,among them,29 were males and 20 were females,with the age ranging from2～79years,average 47 years.Of all the cases,22 were hemangioblastomas,11 were metastatic tumors,7 were astrocytomas,4 were ependymoma,3 were medulloblastomas,1 was epidermoid and 1 was dermoid cyst.Results:The MR detected rate of cerebellar cystic occupying lesions was very high.MR have relative high ability of space location.With MR examination,we can observe not only the morphology of cystic diseases but also the characteristics of signal intensity and edema degree surrounding brain tissue,as well as the change of signal intensity after contrast enhancement.Conclusion:The accurate rate of MR diagnosis in cerebellar cystic occupying lesions is high,specially the enhanced scan has important value in confirming and differential diagnosis.
