口腔颌面外科术中恶性高热的急救处理:附2例报告

摘　　要：目的:探讨口腔颌面外科术中恶性高热的急救处理方法。方法:2例口腔颌面外科手术患者术中突发恶性高热,及时发现并行对症处理。当出现CPK、Mb、LDH极度升高后,及时进行连续性肾脏替代治疗(CRRT)和血浆置换治疗。结果:2例患者经紧急处理及CRRT和血浆置换治疗后,CPK、Mb、LDH显著下降,在渡过了DIC、ARDS及ARF并发症后,均抢救成功出院。结论:对于口腔颌面外科术中恶性高热,及时发现和有效治疗,是抢救成功的关键。PURPOSE： To investigate the emergency treatment of malignant hyperthermia during oral and maxillofacial surgery. METHODS： Two patients with oral and maxillofacial surgery developed malignant hyperthermia during operation. It was detected in time and emergency treatment was given promptly. Continuous renal replacement therapy and plasmapheresis were used when the CPK,Mb and LDH increased significantly. RESULTS： After emergency treatment and continuous renal replacement therapy,CPK,Mb and LDH in the blood plasma decreased significantly and the complication of DIC, ARDS and ARF were resolved .Two patients were successfully cured. CONCLUSIONS： We emphasize early recognition and appropriate therapy are the keys of curing MH successfully.
