摘 要:[目的]探讨放疗对鼻咽癌(NPC)患者垂体功能的短期影响。[方法]通过化学发光免疫分析法和放射免疫分析法检测31例NPC患者治疗前、治疗结束时及治疗结束3个月复查时血清中生长激素(GH)、催乳素(PRL)、黄体生成素(LH)、卵泡刺激素(FSH)及促甲状腺激素(TSH)的浓度。并通过治疗计划系统(TPS)分析患者垂体受照射体积和剂量的关系。[结果]通过TPS计算出30%、50%、80%、100%的垂体体积所受照射剂量分别为(4640.32±2117.01)cGy,(3511.29±2404.04)cGy,(2674.19±2405.47)cGy,(2422.58±2323.28)cGy。不同剂量照射后,复查时5种垂体激素的均值都在正常范围内,但部分激素浓度较治疗前或治疗结束时有显著变化。[结论]鼻咽癌患者放疗后3个月,垂体功能已经发生改变,但并没有发生激素的缺乏。[Purpose ] To evaluate the short-term effect of irradiation on pituitary in patients with nasopharyngealcarcinoma (NPC). [Methods] Serum growthhormone (GH), prolactin (PRL), luteinizing hormone (LH), follicle stimulating hormone (FSH) and thyroid stimulating hormone (TSH) in 31 NPC patients were detected by chemiluminescent immunoassay (CLIA) and radioimmuoassay (RIA) method pre-treatment, finish of treatment and 3 months after treatment. The relationship between the total irradiation dose and the irradiated volume to pituitary were analyzed by the treatment planning system (TPS). [Results] Doses delivered to 30%, 50%, 80%, 100% volume of pituitary were (4 640.32±2 117.01)cGy, (3 511.29±2 404.04)cGy, (2 674.19±2 405.47)cGy and (2 422.58±2 323.28)cGy, respectively. The mean concentrations of 5 types of hormone at 3 month after treatment remained in normal range regardless the dose of pituitary received. However, levels of some hormones changed significantly when compared to those of pre-treatment and finish treatment. [Conclusion] Pituitary function was affected at 3 month after irradiation but without hormones deficiency occurred.
