共同性斜视潜在融合功能的临床研究

摘　　要：目的寻找预测共同性斜视术后能否获得双眼单视的方法。设计回顾性病例系列。研究对象38例9～44岁共同性斜视患者。方法对38例共同性斜视患者于斜视矫正手术前、后分别行同视机、随机点立体图、Bagolini条纹镜、Worth 4点试验及后像法这五项各级融合功能检查,比较各项检查结果。主要指标手术前后各项融合功能的变化。结果术后存在融合功能的人数与术前比较明显增多(P均<0.01)。只有后像试验是各类共同性斜视都能够在术前查到融合的方法。术前5项检查融合的方法中,后像试验检出的融合比例最高,占76.32%,术后获得立体视觉占73.68%。术前具有潜在融合比例与术后获得立体视觉者比例比较,差异无统计学意义(X^2=0.70,P=0.791)。结论9岁以上斜视患者术后双眼单视功能可显著改善。后像灯检查可以作为预测术后共同性斜视能否获得双眼单视的方法。(眼科,2007,16:348-350)Objective To search out the method about binocular single vision in postoperative patients with concomitant strabismus. Design Retrospective case series. Participants 38 patients with concomitant strabismus （9-44 years of age）. Methods 38 patients with concomitant strabismus were examined preoperatively and postoperatively with major amblyoseope, stereogram, Bagolini striated glasses test, Worth Four-Dot test and afterimage test. The outcomes of 5 item methods of fusion examination were compared. Main Outcome Measures Changes of 5 item fusions at preoperation and postoperation. Result The comparison of improvements of fusion cases with 5 item methods at preoperation and postoperation had significant diversity P 〈 0.01. Afterimage test was the sole method that found the fusion in various concomitant strabismus at preoperation. The fusion proportion of afterimage test was the highest （76.32%） of 5 item methods at preoperation. The patients with binocular single vision at postoperation were 73.68%. The diversity of the compare of fusion proportion at preoperation and postoperation was nonsignificant （X^2=0.70,P=0.791）. Conclusion Patients of older than 9 years of age with concomitant strabismus can get the improvement of fusion after operation. Afterimage test can be used as a method for forecast binocuiar single vision of concomitant strabismus preoperatively.
