肝癌肝动脉造影中对比剂对全血黏度的影响

摘　　要：目的探讨对比剂对肝癌介入患者全血黏度的影响。方法肝癌病人53例,随机分为76%复方泛影葡胺组(30例)和碘普罗胺组(23例),分别做体内、体外实验。体内检查在肝动脉造影前、后分别常规采取空腹血标本送检测全血黏度;体外常规采取空腹血标本分成2份,1份加入按体重比例计算的对比剂;1份对照标本,同时送检测全血黏度。结果体内实验:2种对比剂动脉造影术后全血低中高切黏度指标下降,泛影葡胺组下降明显(P<0.05);体外实验:2种对比剂加入静脉血标本后,全血低中高切黏度指标均下降,复方泛影葡胺对高切血黏度影响无显著差异。结论对于血液黏度正常、增高的肝癌患者或伴有肝硬化、门静脉瘤栓形成的病人可以安全应用此2种对比剂;对于血液黏度降低或伴有出血倾向的病人应尽量减少对比剂剂量,建议应用非离子型对比剂。Objective To investigate the influence of contrast medium on the whole blood viscositr in hepatocellular carcinoma （HCC） during hepatic arteriography. Methods 53 patients with HCC were divided into two groups at random：group Ⅰ（ 30 case） and Ⅱ （ 23 cases） ,370 mgⅠ/ml compound meglumine diatrizoate and 370 mgⅠ/ml iopromide were respectively used as contrast medium in arteriography, and intra - body and extra - body experiment were respectively done. In intra - body test,the index of the whole blood viscosity （ BV ） in the state of emply stomach was measured before and post hepatic arteriography in both group Ⅰ and Ⅱ. In extra - body test, two fasting blood samples were collected, one sample was added by contrast medium according to the proportion of body weight, and another one was as controller,and BV index was also measured. Results In intra - body test,the low and middle shear whole BV index was decreased significantly （ P 〈 0.05 ） after hepatic arteriography using two contrast medium , but using compund meglumine diatrizoate was. In extra - body test, the low , middle and high shear,the whole BV index was all decreased using two contrast agents and there was no significant difference. Conclusion Two contrast agents can be used saftly in the patients with normal and high BV,including the patients with cirrhosis and tumor thrombosis in portal vein; two contrast agents should be used as less as possible in the patients with low BV and the patients extending to bleeding,iopromide is suggested.
