摘 要:目的研究血管生成素-1(Angiopoietin-1)及血管内皮生长因子(VEGF165)协同作用对急性下肢缺血兔模型血管再生的作用。方法日本大耳兔53只,结扎离断股动脉,建立兔急性下肢缺血模型,在缺血肢体转染phVEGF165250 ug 14只、phAng1500 ug 14只、phVEGF165250 ug+phAng1 500 ug 14只、PBS液11只(对照组),转染后3、7、14、30 d取转染部位骨骼肌,检测基因表达和毛细血管密度;治疗30 d后观察肢体缺血、侧枝血管形成和评价肢体血流。结果VEGF165+Ang1治疗组动物肢体坏死发生率低于VEGF165、Ang1单独治疗组和对照组(分别是18.2%、45.5%、27.3%和100%);30 d后缺血肢体最大血流速率4组无差别(P>0.05),VEGF165+Ang1治疗组血管计数高于单独治疗组和对照组(q=61.2034,q=54.6494,q=50.0568,P<0.01),毛细血管密度高于单独治疗组和对照组(q=227.7980,q=144.0765,q=100.2765,P<0.01)。结论Ang1可促进兔急性缺血肢体对VEGF血管再生的反应,经肌肉给予细胞因子促进兔缺血肢体血运重建是可行的。Objective To explore the co - operative effect of angiopoietin - 1 ( Ang1 ) and vascular endothelail growth factor( VEGF165 ) in angiogenic response in rabbit model of acute hindlimb ischemia. Methods Creat rabbit model with acute hindlimb ischemia in 53 rabbits by ligation of the external iliac artery and the excision of the common and superficial femoral artery, 250ug of phVEGF165 ( n = 14 ) ,500 ug of phAng1 ( n = 14 ) ,250 ug of phVEGF165 plus 500 ug of phAng1 ( n = 14 ) and PBS( control group, n = 11 ) was injected intramuscularly into the ischemic hindlimb muscles. The transfected muscles were harvested 3,7,14,30days later, detecting the expression of VEGF165 or Angl and investigating capillary density stained by Ⅷ. Gross appearance of ischemic limb, collateral vessel formation and limb perfusion were assessed 30 days after treatment. Results The incidence of ischemic limb necrosis was higher in the animals treated by VEGF165 ,by Ang1 or by PBS than that in those treated by VEGF165 plus Ang1( 18.2% ,45.5% ,27.3% and 100% ,respectively). Animals in four group had no significance in maximum calf blood velocity at day 30 ( P 〉 0. 05 ) . A combination therapy of VEGF plus Ang1 had a significantly higher ( q = 61. 2034,q =54. 6494 , q =50. 0568 ,P 〈0.01 ) angiographic score than either therapy alone or control group . Capillary density (q =227. 7980, q = 144. 0765, q = 100. 2765 ,P 〈 0. 01 ) in the combination therapeutic group was also significantly higher than that of either therapy alone or control group. Conclusion Ang1 can potentiate the angiogenic response to VEGF in rabbit model of acute hindlimb ischemia. Intramuscular administration of cytokines on revascularization of the ischemic hindlimb model of rabbit is feasible.
