摘  要:目的探讨微波或碘-125粒子治疗兔VX2肝癌与两者联合治疗的不同疗效。方法将80只单点种植VX2肝癌瘤株的新西兰大白兔,B超动态监测肿瘤大小2 cm时,随机分入四组,每组20只,A组,微波治疗组(40 W功率作用2min);B组,碘-125粒子治疗组(0.5 mCi,肿瘤周边处方剂量60 Gy);C组,微波和碘-125粒子联合治疗组;D组,未治疗的对照组。治疗前均行CT平扫及增强扫描,制定治疗计划,A组、B组和C组在CT引导下经皮穿刺治疗兔VX2肝癌,治疗后两天摄腹部平片验证,三周末切除肿瘤标本测量肿瘤体积大小,进行统计学分析,同时取肿瘤组织制成病理切片,分别采用HE染色,观察其病理变化,并取肿瘤在电镜下观察其超微结构变化。结果A组,肿瘤体积大小为(33.6±12.3)cm3,B组,肿瘤大小为(35.1±13.2)cm3,C组,肿瘤大小为(6±2.3)cm3,C组联合治疗组与A组微波治疗组和B组碘-125粒子治疗组比较有统计学意义(P<0.05)。D组,肿瘤大小为(198.1±72.1)cm3,治疗组(A组、B组和C组)与未治疗的对照组(D组)比较亦有明显的统计学意义(P<0.01)。结论经皮微波和碘-125粒子联合治疗兔VX2肝癌,可以弥补单独治疗组的不足,疗效可靠,是一种新的有效的联合治疗手段。Objective This study was undertaken to explore a combination therapy with percutanous microwave coagulation therapy (PMCT) and ^125I seed brachytherapy for liver cancer, to determine if it can increase the therapy efficiency compared with PMCT alone or ^125I seed brachytherapy alone in a Rabbit VX2 tumor Model. Methods Eighty New Zealand white rab bits were implanted with VX2 tumor in the deep aspect of the left lobe of liver. After monitoring with B-ultrasound that 2 cm diameter tumors were observed, rabbits were classified into four groups. Initially, tumor were treated with (a) PMCT (power at 40 W for 2 minutes) alone, (b) 1^25I seed brachytherapy alone. The activity of per seed was 0.5 mCi. The peripheral radiation total dose was 60 Gy, (c) the combination therapy with PMCT and ^125I seed brachytherapy, (d) no treatment. The rabbits were examined by CT for plan before treatment. (a), (b) and (c) CT-guide percutanous treatment and taken plain film for prove the plan after 2 days after treatment. Tumors were measured after 3 weeks treatment by excision and done pathologic examination with H&E dye and done electron microscope examination. Results Mean volume for treated tumors were as following: (a) (33.6±12.3) cm^3, PMCT alone; (b) (35.1 ±13.2) cm^3, 1251 seed brachytherapy alone. (c) (6±2.3) cm^3 , the combination therapy (P〈0.05, compared with PMCT alone or ~2sI seed brachytherapy alone);(d) (198.1± 72.1) cm^3 ,no treatment (P〈0.01, compared with treatment group a,b and c). Conclusion The combination therapy with PMCT and ^125I seed brachytherapy for liver cancer in the rabbit VX2 tumor model is a new combination therapy with good effect by making up the deficiency of PMCT alone or ^125 I seed brachytherapy alone.
