γ探测仪结合核素淋巴显像在皮肤恶性黑色素瘤前哨淋巴结检测中的应用

摘　　要：目的：探讨γ探测仪结合核素淋巴显像检测皮肤恶性黑色素瘤前哨淋巴结（sentinel lymph node,SLN）的准确性及其临床价值。方法：27例皮肤恶性黑色素瘤患者,术前1-2h在肿瘤病灶周围3、6、9、12点钟位置皮下注射显像剂^99mTc-硫胶体,用γ探测仪结合核素淋巴显像定位探测SLN,术中切除SLN,随后行淋巴结清除术。对阴性SLN进行连续切片,行S-100和HMB-45免疫组化分析。结果：26例患者检出SLN,检出率为96.2%,SLN阳性为7例,阳性率为26.9%,准确率为100%。结论：γ探测仪结合核素淋巴显像检测恶性黑色素瘤的SLN是准确的,能够反应区域淋巴结转移状态,能很好的协助SLN的定位。OBJECTIVE： To evaluate the accuracy and clinical value of gamma probe and lymphoscintigraphy in sentinel lymph node identification for cutaneous elanoma. METHODS.^99mTc-labeled sulfur colloid was injected in 27 patients with cutaneous melanoma about 1-2 hours before the operation. The sentinel lymph nodes were identified by lymphoscintigrphy and gamma probe, followed by resection and regional lymphadenectomy. Serial section and immunohistochemistry staining with antibodies to S-100 and HMB-45 were peformed in all negtive sentinel lymph nodes. RESULTS： Sentinel lymph nodes were identified in 26 patients, with a detection rate of 96.2%. Seven （26.9%） of twenty-six melanomas were found to have positive sentinel lymph nodes. The accuracy rate of sentinel lymph node was 100%, CONCLUSION.. Gamma probe and lymphoscintigraphy aided sentinel lymph node biopsy can predict regional node status.
