彝汉族聚居地区胃Hp感染的血清流行病学研究

摘　　要：目的了解我国少数民族地区Ｈｐ感染是否存在民族差异．方法１９９４年４月－７月用整群抽样收集和调查云南云县彝族（ｎ＝４６６）、汉族（ｎ＝６１８）聚居地区１０８４名自然人群．作血清抗Ｈｐ－ＩｇＧ检测，其中３２５人同时作血清抗ＨＡＶ－ＩｇＧ检测．同时进行问卷调查有关流行因素．结果彝族Ｈｐ感染单因素分析与饮水等１０个流行因素相关，多因素Ｌｏｇｉｓｔｉｃ分析筛选出饮水、养猪、家族胃癌史、大蒜和吸毒等６个重要因素．汉族Ｈｐ感染单因素分析与饮水等１４个流行因素有关，多因素Ｌｏｇｉｓｔｉｃ分析筛选出饮水、上消化道症状、吸毒和大蒜等５个重要因素．彝、汉族感染率无差异，作４８个变量彝、汉族的相关分析除家族胃癌史和服用中药有显著性差异外均无显著性差异．AIM To study Hp infection situation and the ethnic difference in the minority regions. METHODS From April to July 1994, a group sampling survey was made in a natural population of 1084 people across a Yi_Han inhabited region at Yunxian County of Yunnan Province. Serum anti_Hp_IgG was determined in the 1084 people, and 325 of them were given a simultaneous measurement of serum anti_HAV_IgG. High risk factors were also from each people by questionaires. RESULTS Among the Yi people the single_factor analysis showed that Hp infection was associated with 10 epidemic factors, while from the multi_factor logistic analysis it was associated with 6 major factors such as drinking water, pig_raising, family history of gastric cancer, garlic and drug abuse. Among the Han people the single_factor analysis indicated that Hp infection was related to 14 epidemic factors such as drinking water whereas to 5 important factors by the multi_factor logistic analysis such as drinking water, upper digestive_track symptom, drug_taking, and garlic. No difference was observed in Hp infection between the Yi and Han nationalities. The analysis of 48 variables revealed no significant difference between Yi and Han, except a marked difference in family history of gastric cancer and intake of traditionl Chinese medicines. CONCLUSION No significant difference was found in Hp infection between Yi and Han nationalities.
