抑郁症患者甲状腺功能与植物神经功能的关系

摘　　要：目的研究抑郁症患者甲状腺功能和植物神经功能的变化。方法研究我院门诊患者单相抑郁症38例,双相抑郁症组42例,正常对照组30例。用汉密顿量表(17项版本)进行精神测评。用化学发光法测定血清FT3、FT4、TSH的水平,用植物神经平衡指数来评定植物功能。结果与对照组比较,单相抑郁症患者血清FT3水平下降,FT4水平上升,TSH水平无变化,双相抑郁症患者血清FT3水平下降、FT4水平上升更显著(P<0.01)。单相抑郁和双相抑郁患者植物神经功能评定均提示交感神经活性增强。与轻度抑郁症相比,重度抑郁症患者血清FT3水平下降、FT4水平上升和植物神经平衡指数上升更显著(P<0.01)。结论抑郁症患者交感神经活性增强,其血清FT3水平下降、FT4水平上升。Objective To study the changes of thyroid function and automatic nervous function in patients with depression. Methods All patients were divided into three groups ：38 cases with unipolar depression,42 cases with bipolar depression, and 30 cases with norm. Hamilton depression scale （HAMD） and Hamilton anxiety scale （HAMA） were used to assessed psychological status. Thyroid function （ FT3 , FT4 ,TSH） were measured by chemolumminescence method. The function of automatic nervous system was showed by balance index of automatic nerve. Results Compared with control group, unipolar depression group had significant lower level of FT3 , and higher level of FT4. However ,the change of TSH between two groups was not significantly different. In bipolar depression group, the level of serum FT3 significantly dropped, and FT4 rose remarkably （P 〈 0.01 ）. The function of automatic nervous system in both unipolar depression and bipolar depression indicated dominant sympathetic activity. Severe depression group had markedly higher FT3 level,lower FT4 level,and higher balance index of automatic nerve than mild depres- sion group （P 〈 0.01 ）. Conclusion Patients with depression have dominant sympathetic activity, higher FT3 level and lower FT4 level.
