脂肪源间充质干细胞联合化疗治疗多发性骨髓瘤骨痛的初步研究

摘　　要：目的 评价脂肪源间充质干细胞（AMSC）联合化疗治疗多发性骨髓瘤（MM）病人骨痛的疗效。方法 经确诊为MM的患者20例,随机分为治疗组和对照组。治疗组10例MM患者,在接受M2方案化疗结束后48 h,以每公斤体重2×105个细胞剂量静脉输注无关供者AMSC;对照组给予M2方案化疗;以骨痛的缓解程度和血钙下降度作为评价指标。结果 治疗组和对照组用药前均有骨痛,治疗两周期后,治疗组完全缓解7例,部分缓解3例,有效率100%;对照组完全缓解2例,部分缓解2例,有效率40%,治疗组有效率高于对照组（P〈0.05）。治疗组血钙升高8例,治疗后均降至正常范围,下降率为100%;对照组血钙升高9例,治疗后4例降至正常范围,下降率44.4%。毒副作用方面：治疗组病人在使用AMSC过程中均未见有明显不良反应,病人耐受性良好。结论 脂肪源间充质干细胞联合化疗可显著缓解多发性骨髓瘤病人的骨痛。Objective To study the therapeutic effectiveness of adult adipose tissue -derived mesenchymal stem cells （AM- SC） combined with chemotherapy for bone pain in multiple myeloma patients, Methods In this randomized, controlled study, 20 patients with pathological osteolytic lesion induced by myeloma were enrolled, Patients were randomly divided into two groups. Ten cases in the treated group A and the other 10 patients in the control group B, Forty - eight hours later after being treated with chemotherapy （M2）, a dose of 2 × 10^5 AMSC per kg of the patients＇weight was given intravenously in group A, Patients in group B were treated with chemotherapy （M2） only. Pain relief and the level of serum calcium were observed. Results The percent of pain relief in the two groups were 100% （group A） and 40% （ group B）, respectively. All patients with hyperc＇alcaemia （8/8） were corrected in the group A after two -cycle treatment compared to those were 44, 4% （4/9） in group B, Pain relief and the control of hypercalcaemia in group A were superior to those in group B, In addition, no AMSC infusion - related side - effects were noted. Conclusion Adult adipose tissue - derived mesenchymal stem cells combined with chemotherapy are effective in relieving bone pain in patiens with multiple myeloma.
