C-臂机引导下经皮半月节穿刺甘油阻滞术治疗原发性三叉神经痛疗效分析

摘　　要：目的 探讨采用C-臂机引导下经皮穿刺三叉神经半月节甘油阻滞术治疗原发性三叉神经痛的临床疗效。方法 患者仰卧位,在C-臂机引导下,采用Hartel前入路穿刺法,局麻下经卵圆孔穿刺三叉神经半月节,穿刺成功后向半月节内注入甘油0.4～0.5 ml。结果 三叉神经半月节甘油阻滞术治疗31例患者,一次性治疗疼痛消失24例,部分缓解5例,无效2例,总有效率达93.5%。随访28例,随访时间平均为7个月～3.3年,9例复发,总复发率为32.1%。结论 采用C-臂机引导下经皮穿刺三叉神经半月节甘油阻滞术治疗原发性三叉神经痛创伤小、安全性高,是高龄或不能耐受开颅手术病人的首选治疗方法 。Objective To investigate the treatment of trigeminal neuralgia by injection of the gasserian ganglion using C - Arm guidance. Methods Percutaneous retrogasserian glycerol instillation using C - Arm guidance was performed under local anesthesia for treatment of trigeminal neuralgia in 31 patients. Results Initial pain relief was achieved in 24 patients （77.4%）. 29 （93. 5 % ） of the original 31 patients experienced satisfactory pain control following glycerol rhizolysis. The follow - up period extended from 7 months to 3.3 years in 28 patients and recurrent pain was noted in 9 patients（ 32.1% ）. Conclusion Pereutaneous retrogasserian glycerol rhizolysis offers a rapid, safe,reliable and relatively inexpensive surgical approach to treatment of trigeminal neuralgia.
