摘  要:学术背景:目前神经系统许多疾病仍无有效治疗方法,应用干细胞治疗神经损伤,使神经元再生和神经功能恢复成为可能。骨髓间充质干细胞因具有自我增殖和多向分化能力,成为干细胞疗法治疗神经系统疾病的最佳选择之一。目的:探讨骨髓间充质干细胞在治疗神经损伤疾病方面的应用及其神经修复保护的作用机制。检索策略:由该论文的研究人员应用计算机检索Pubmed数据库1997-01/2007-01的相关文献,检索词"bone marrow stromal cells,nerve injury,transplantation,neuroprotective",并限定文章语言种类为English。同时计算机检索中国期刊全文数据库2002-01/2007-01的相关文献,检索词"骨髓间充质干细胞,神经损伤,移植,神经保护",并限定文章语言种类为中文。共检索到250篇文献,对资料进行初审,纳入标准:①与骨髓间充质干细胞治疗神经损伤应用及神经修复保护研究相关。②同一领域选择近5年且发表在较权威杂志者。排除标准:①重复性研究。②Meta分析。文献评价:文献的来源主要是通过对骨髓间充质干细胞在治疗神经损伤疾病方面的应用及其神经修复保护作用机制进行汇总分析。所选用的30篇文献中,1篇为综述,其余均为临床或基础实验研究。资料综合:①骨髓间充质干细胞不仅能够自我更新和多向分化,且具有跨胚层横向分化能力,可通过细胞表面标志并结合细胞形态对其进行初步鉴定。②骨髓间充质干细胞分泌多种神经生长因子作用于神经元相应受体,通过受体信号传导系统激活生存信号、失活死亡信号,从而启动分化机制,减少神经细胞凋亡,诱导内源性细胞增生,并促进神经纤维再生,是其产生神经细胞保护作用的主要机制。③骨髓间充质干细胞临床应用目前主要涉及脑血管疾病、脊髓损伤、神经系统变性疾病等。动物实验显示经血管、腹腔等不同途径移植后,骨髓间充质BACKGROUND: At present, there are no effective strategies in the treatments of nervous system diseases. The application of stem cells in treating nerve injury has made it possible to promote neuronal regeneration and functional recovery. Bone marrow mesenchymal stem cells (BMSCs), which have the potential of self-renewing and multi-differentiation, have been regarded as one of the best therapies in nervous system diseases. OBJECTIVE: To explore the application and neuroprotective mechanism of BMSCs grafting in the treatment of nerve injury. RETRIEVE STRATEGY: An online search of Pubmed Database was undertaken to identify articles published from January 1997 to January 2007 in English by using the keywords of "bone marrow stromal cells, nerve injury, transplantation, neuroprotective". At the same time, China Journal Full-text Database (CNKI) Database was searched to identify the related articles from January 2002 to January 2007 with the same keywords in Chinese. A total of 250 literatures were selected primarily. Inclusive criteria: (1)articles on BMSCs transplantation in the treatment of nerve injury and effects of neuroprotection, and (2)articles mainly focused on the magazines published in recent 5 years. Exclusive criteria: (1)rapeated study, and (2)Meta analysis. LITERATURE EVALUATION: Article resources about BMSCs application in the treatment of nerve injury and mechanism of neuroprotection were analyzed. Of 30 selected articles as the references, all were about clinical or basic studies except 1 review. DATA SYNTHESIS: (1)BMSCs have the potential of self-renewing and multi-differentiation, they can transdifferentiation, and can be primarily identified by cell surface marker combined with cell appearance. (2)BMSCs can differentiate into many nerve growth factors acting on corresponding neuron receptor, activate survival signal, devitalize dead signal, so as to start differentiation, decrease apoptosis, induce endogenous cellular proliferation, and promote ne
