规范化治疗对急性脑卒中早期疗效的影响

摘　　要：目的探讨规范化治疗方案对急性脑卒中患者的早期疗效。方法由我院脑血管病研究所专家制定急性脑梗死和脑出血规范化诊断和治疗方案，内容包括建立脑血管病绿色通道方案、卒中小组人员组成方案、诊断方案、综合治疗方案、发病不同时间段治疗方案、护理方案、康复训练方案等。收集2004年5月～2006年4月住院的急性脑梗死或脑出血患者共265例，根据制定的统一方案进行诊治。以2002年1月～2003年12月同条件的脑梗死或脑出血患者282例为对照。比较两组间主要临床指标。结果虽然规范化治疗组和对照组住院时间和住院费用差异无显著性（P〉O．05），但规范化治疗组住院病死率低（P〈0．05），肺部感染发生率低，神经功能改善明显（P〈0．01）。结论相比对照组，规范化治疗能减少脑卒中住院患者的病死率。明显提高患者的早期日常生活能力，减少神经功能缺损，提高回归社会的能力，不增加患者的花费。Objective： To investigate the early standard-treatment effect on standard - treatment the acute stroke patients. Methods： The standard diagnosisand therapy were performed by the neurological special of our department, which including the stroke units, standard treatment ,nursing and rehabilitation, etc. 265 patients with acute stroke were recruited into study group from May 2004 to April 2006. 282 of acute stroke patients from January 2002 to December 2003 were recruited into control group. Their clinical data were analyzed. Results： Although there was no statistical difference of hospitalization stay and cost between the two groups （P〉0.05）, the death rate of the control group was higher than that of the study group（P〈0.05）, and the pulmonary affection was reduced obviously in the study group（p〈0.01）.Conclusion： compared with control group, standard treatment can reduce the stroke patients＇ death rate, improve their daily activity in early stage, reduce neurological defects ,restore social ability without increased spending.
